Schedule Full Time, Day Shift (80 hours biweekly) Hours: Monday - Friday, 8:00am - 4:30pm General Summary Provides quality clinical mental health assessments and consultation to providers and supporting clinical care team staff as necessary. They are responsible for triage and management of patients with clinical issues related to their behavioral or psychosocial wellbeing. Additionally, the behaviorist conducts psychosocial assessments, interventions, Transitions of Care (TOC), education, and referrals to inform a holistic treatment plan for patients, in support of Population Health. They are responsible for the management and oversight of a caseload of moderate - high risk Members with complex medical/behavioral /psychosocial needs. Remote Work Capable This is a hybrid position. The applicant must reside within the WellSpan blueprint to be within 30-45 minutes of a touchdown location. (York/Chambersburg/Gettysburg area) Essential Functions: Conduct thorough biopsychosocial assessments to determine patient needs and appropriate interventions. Utilize standardized tools and clinical interviews to assess mental health conditions, substance abuse, and social determinants of health. Provide evidence-based individual, group, and family therapy to address mental health and behavioral issues. Develop and implement personalized care plans with measurable goals and objectives. Utilize therapeutic techniques such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Motivational Interviewing (MI). Coordinate care and services for patients, including referrals to community resources, follow-up, and continuity of care. Collaborate with multidisciplinary teams, including psychiatrists, psychologists, nurses, and primary care providers. Identifies, refers and advocates for patients requiring specialty behavioral health services and other services as needed. Maintain accurate and timely documentation of assessments, treatment plans, progress notes, and discharge summaries in compliance with regulatory and organizational standards. Ensure all practices comply with ethical standards, state licensure requirements, and HIPAA regulations. Provide education to patients and families about mental health conditions, treatment options, and coping strategies. Assists in the detection of “at risk” patients and the development of plans to prevent further psychological or physical deterioration. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise. Qualifications Minimum Education: Masters Degree Graduate of an accredited school of social work with a Master's Degree in Social Work. Required Work Experience: 2 years Clinical recent/related experience; care management, home health, medical/surgical behavioral health, and/or critical care. Additional experience related to specialized positions (Oncology, Transplant, Maternity, Behavioral Health) will also be considered. Preferred and 2 years Experience in social work counseling with experience in complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background. Preferred and Less than 1 year Experience in health coaching and motivational interviewing techniques. Preferred Licenses: Licensed Clinical Social Worker Upon Hire Required Knowledge, Skills, and Abilities: Stay current with developments in the field of social work, behavioral health, and mental health treatment. Participate in continuing education, supervision, and peer consultation to maintain licensure and enhance clinical skills. Competency in providing cognitive and behavioral intervention to individuals, families, couples and groups with behavioral health/substance abuse problems. Ability to work effectively in a multidisciplinary team. Excellent communication, organizational, and problem-solving skills. Familiarity with electronic health records (EHR) and documentation standards.
