Position Description:

Manages the development, implementation, and evaluation of population health outreach, education, engagement, care coordination, and clinical quality improvement initiatives designed to improve oral health outcomes, increase dental benefit utilization, and strengthen relationships with members, providers, community organizations, and key stakeholders. Oversees outreach programs and staff, drives quality performance, monitors clinical outcomes, and advances organizational goals related to health equity, preventive care, and member engagement.


Primary Job Responsibilities:

  • Manages the planning, development, implementation, and evaluation of population health and quality strategies, including targeted initiatives for underserved and at-risk populations, to improve access to care, preventive service utilization, health equity, and oral health outcomes.
  • Analyzes quality and utilization data to identify trends, disparities, performance gaps, and target populations, and recommends outreach, engagement, and quality improvement priorities.
  • Develops and executes outreach plans and programs for Medicaid, Medicare, and other government-sponsored populations to improve access to care, preventive service utilization, member engagement, and quality performance.
  • Oversees care coordination and quality performance staff, including hiring, coaching, performance management, professional development, and workload prioritization, to ensure effective program delivery and achievement of department goals.
  • Provides subject matter expertise on population health, oral health education, community engagement, care coordination, preventive health initiatives, and quality measures to internal teams and external stakeholders.
  • Develops, implements, and monitors outreach policies, procedures, standards, and best practices to ensure program effectiveness and operational excellence.
  • Manages budget, resources, contracts, and outreach program activities to maximize impact and support organizational objectives.
  • Oversees the creation, review, and distribution of educational materials, presentations, and health promotion campaigns for members, providers, employers, and community partners.
  • Collaborates with the data analysis team to define and monitor program metrics, evaluate performance, develop reports and recommendations, and identify opportunities for continuous improvement.

Perform other related assigned duties as necessary to complete the Primary Job Responsibilities as described above.


Minimum Requirements:

Position requires a bachelor’s degree in dental hygiene, public health, health education, health administration, or a related field, three years of experience in dental hygiene, public health, population health, outreach, care coordination, community health, or related healthcare programs, including experience working with clinical quality programs and initiatives, and two years of leadership experience. A quality professional certification, such as CPHQ or HCQM, is preferred.

Position requires strong knowledge of population health management, preventive healthcare, health equity, and community outreach principles; ability to analyze and interpret data to develop actionable outreach strategies; strong presentation, facilitation, and public speaking skills; strong relationship-building and stakeholder engagement abilities; ability to collaborate effectively across departments and with external partners; strong project management and organizational skills; strong written and verbal communication skills; and ability to work independently, manage multiple priorities, and achieve results in a team-oriented environment. Knowledge of dental terminology, dental benefits, procedure coding, and healthcare delivery systems is preferred.
