Overview Ansible Government Solutions, LLC (Ansible) is seeking multiple Medical Records Technicians (Coders) to work with us in support of the VA Northeast Ohio Healthcare System (Cleveland VA Medical Center, Wade Park Campus) located at 10701 East Blvd, Cleveland, OH 44106. This is primarily a remote work environment and the shift schedule typically ranges Monday-Friday, 6:00 AM - 6:00 PM. Responsibilities Perform coding of inpatient, outpatient, surgery, prosthetics, diagnostic testing, endoscopy, and professional services encounters. Assign ICD-10-CM, ICD-10-PCS, CPT, HCPCS, and E/M codes. Ensure accurate code assignment, sequencing, and documentation support. Utilize Oracle Health and Solventum CRS Encoder for coding functions Review medical records for complete and accurate provider documentation. Identify documentation deficiencies, inconsistencies, or conflicts. Submit provider queries when clarification is required. Verify medical necessity and documentation integrity. Apply coding standards according to: ICD-10-CM ICD-10-PCS CPT HCPCS E/M Guidelines National Correct Coding Initiative (NCCI) VHA HIM Coding Program Standards Ensure all coding complies with Federal, VA, and VHA requirements Submit compliant provider queries when documentation is incomplete, conflicting, or ambiguous. Track and resolve queries through approved VA processes. Close queries within 30 calendar days of discharge/service unless otherwise directed. Meet or exceed: 3 inpatient PTFs per day 25 outpatient/prosthetic encounters per day 9 surgical encounters per day Participate in coding audits and corrective action activities. Maintain coding accuracy standards. Prepare monthly quality assurance and improvement reports. Participate in meetings with HIM leadership and COR. Qualifications and Requirements Must possess at least one active credential from AHIMA or AAPC (must remain active throughout contract performance): RHIT RHIA CCS CCS-P CPC COC CIC Minimum of two (2) years of continuous coding experience in a Level 1A VA Medical Center or equivalent facility. Minimum of two years coding using: ICD-10-CM ICD-10-PCS CPT HCPCS coding systems. Must demonstrate proficiency in: ICD-10-CM ICD-10-PCS CPT HCPCS Evaluation & Management (E&M) Oracle Health Solventum CRS Encoder Provider query processes Medical terminology Anatomy and physiology Pharmacology Pathology and disease processes Documentation review and abstraction techniques.
