Do you want work that makes a meaningful difference in your community? Easterseals PORT Health is seeking a Mobile Crisis Coordinator to join our VA REACH Team serving Central Virginia and the Shenandoah Valley.
The Community Regional Crisis Response Team provides immediate, community-based behavioral health crisis intervention for individuals experiencing mental health and substance use crises. Through rapid assessment, stabilization services, and coordination with local resources, the team helps people remain safely in their communities whenever possible while reducing unnecessary hospitalizations and emergency department visits. This critical service supports individuals and families during some of life's most challenging moments and creates pathways to recovery and long-term stability.
As a Mobile Crisis Coordinator, you'll respond directly to individuals in crisis, conduct comprehensive assessments, develop treatment and safety plans, and provide short-term therapeutic interventions. You'll work alongside families, community providers, and multidisciplinary teams to coordinate care and connect individuals with the resources they need to thrive.
What You'll Do
- Provide mobile crisis assessments, interventions, stabilization services, and short-term counseling in community settings.
- Partner with individuals, families, healthcare providers, first responders, and community stakeholders to support crisis resolution and recovery.
- Coordinate treatment recommendations, referrals, service plans, and follow-up care.
- Document assessments, treatment plans, interventions, and client outcomes in accordance with regulatory and agency requirements.
- Facilitate support meetings and collaborate with multidisciplinary teams to ensure comprehensive, person-centered care.
- Contribute to quality improvement efforts and positive client outcomes through effective crisis response and care coordination.
- Participate in the program's rotating on-call schedule as assigned.
What You Bring
We're looking for someone who brings the experience, skills, and commitment needed to succeed in this role.
- Master's degree in a license-eligible counseling, social work, marriage and family therapy, or related clinical field.
- REQUIRED: License-eligible as an LPC, LCSW, or LMFT in Virginia.
- Minimum of one year of experience working with mental health populations preferred.
- Strong crisis intervention, adaptability, critical thinking, problem-solving, and decision-making skills.
- Experience collaborating within multidisciplinary treatment teams preferred.
- Proficiency with Electronic Health Records (EHR), Microsoft Office Suite, and related technology platforms.
- Valid driver's license, acceptable driving record, current auto insurance, and ability to travel throughout the service area.
