Position Purpose The Field Nurse Case Manager plays a critical role in facilitating and coordinating the medical rehabilitation needs of injured workers in a timely and cost-effective manner. This position requires the consistent use of clinical expertise, independent judgment, and discretion to promote optimal outcomes. You will advocate for the injured worker's physical and emotional well-being, coordinate multidisciplinary resources, and implement strategies that significantly influence a safe and effective return-to-work process. Core Responsibilities Clinical Assessment & Planning: Independently assess the injured worker's medical condition and develop a coordinated, individualized treatment plan in collaboration with healthcare providers. Field Coordination: Coordinate and attend medical appointments as needed to ensure continuity of care, provide support, and align all parties with the treatment plan. Stakeholder Communication: Maintain timely and professional communication with all key stakeholders, including injured workers, medical professionals, employers, and insurance/claims representatives. Return-to-Work Strategy: Collaborate with employers to evaluate, recommend, and implement job duty modifications based on medical restrictions and functional capacity assessments. Patient Advocacy & Education: Interpret and explain complex medical instructions from physicians and therapists to injured workers to ensure full understanding and compliance. Serve as a liaison and advocate throughout the recovery process. Strategic Intervention: Analyze client needs and recommend strategic clinical interventions that positively impact overall case outcomes, regulatory compliance, and resource allocation. Documentation & Reporting: Prepare and submit detailed, accurate monthly progress reports and document all time and activities in accordance with company standards and regulatory guidelines. Industry Engagement: Cultivate and maintain strategic business relationships to emphasize proactive engagement. Stay current with developments in medical case management, occupational health, and workers' compensation regulations. Requirements Licensure: Valid and unrestricted Registered Nurse (RN) license required. Certification: Certified Case Manager (CCM), Certified Occupational Health Nurse (COHN), or Certified Rehabilitation Registered Nurse (CRRN) is highly preferred. Experience: Prior experience in medical case management is required. Direct experience in workers' compensation, occupational health, or disability case management is strongly preferred. Experience in hospital, orthopedic, or rehabilitation settings is a plus. Skills: Strong understanding of medical terminology, clinical practices, and workers' compensation laws. Demonstrated ability to exercise independent clinical judgment and make decisions that impact case outcomes. Excellent interpersonal, negotiation, and communication skills (written and verbal). Strict adherence to HIPAA regulations and confidentiality standards. Other Requirements: Ability to travel locally for field appointments (valid driver’s license and reliable transportation required). Multilingual proficiency is a plus. Benefits Why Apply?
