About the Role Remo Medical Group, P.A., the affiliated medical practice of Remo Health ("Remo"), is hiring a remote Advanced Practice Clinician. Remo is seeking a Psychiatric Mental Health NP (PMHNP) or Adult Gerontological NP (AGPCNP) who is dedicated to the assessment and longitudinal care of patients living with dementia. This role operates within an Interdisciplinary Team framework, requiring specific expertise in dyad care - supporting both the patient and their caregiver - alongside our robust longitudinal care navigation model of care that aligns clinical decisions with What Matters Most to the family. We are looking for a clinician who thrives in a fast-paced startup environment and is excited about operating at the intersection of medicine and technology. As an Advanced Practice Clinician you will evaluate and manage a panel of patients with cognitive disorders and their caregivers (dyads) as well as be an integral part of the interdisciplinary team which includes Neurologists, Geriatricians, Care Navigators, Registered Nurses, Nurse Practitioners, Licensed Clinical Social Workers, and Pharmacists. You will report directly to our Medical Director and work closely with them on hiring and training new providers as we expand to new markets. Remo clinicians help to continually define how our organization serves patients, families, and care team members, and work cross-functionally to ensure that our processes and tools are built to support individuals living with dementia and their family caregivers. What You’ll Be Doing Clinical Practice & Operations: Diagnostic Evaluations: Perform comprehensive assessments to establish, confirm and stage dementia diagnoses. Care Plan Formulation: Drive the creation and implementation of evidence-based Care Plans covering clinical, behavioral, safety and caregiver support goals. Dementia Proficiency: Stay current with the latest advancements in dementia care, translating and sharing this expertise across the team to ensure the highest quality of clinical care. Quality Improvement: Proactively identify opportunities to optimize the Care Model and the technical platform, directly enhancing dyad satisfaction, clinical quality and the overall provider experience. Care Delivery: Longitudinal Support: Manage a panel of patients living with dementia and their caregivers over time, partnering seamlessly with the Interdisciplinary Team. Dyad-Centric Care: Evaluate the distinct needs of the patient-caregiver unit, anchoring care interventions around “What Matters Most” to them. Crisis Prevention: Proactively monitor for cognitive changes and environmental stressors to minimize acute escalations, unnecessary emergency department visits and hospitalizations. Cross-Functional Collaboration: KPI Alignment: Partner closely with teams to achieve critical operational benchmarks, including clinical quality targets, utilization metrics, and patient/provider satisfaction. 24/7 Care Access Support: Participate in structured operational workflows that ensure that our dyads have reliable access to care support 24/7. You May Be a Good Fit If You Clinical & Educational Requirements: Certification: Board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) or Adult-Gerontological Nurse Practitioner (AGPCNP-BC) Education: Graduate of an accredited US Nursing School and an accredited Nurse Practitioner Program Speciality Experience: Proven clinical background in Geriatrics, palliative care or memory care, with deep experience in dementia and related cognitive disorders. Communication Skills: Demonstrated experience in advance care planning and serious illness communication with patients and their caregivers. Teamwork: Thrive working alongside an interdisciplinary care team to deliver holistic, dyad-centered care. Technical & Environment Fit: Digital Health Experience: Prior experience thriving within a high-growth startup or digital health environment Remote Excellence: excel in communicating in a Remote-first environment, comfortably embracing asynchronous tools (Slack, Notion, etc) while fostering a remote culture with warmth and clarity. Telehealth Comfort: Enjoy delivering high-quality care via a telehealth platform Mindset: Bring strong problem-solving skills and a flexible, creative approach to navigating the unique challenges of startup workflows and complex patient care. You’re the Ideal Candidate If You Have Licensure: An active, unrestricted license in OR, WA, ID, and OH. Teamwork: A proven track record of working effectively with interdisciplinary teams Leadership: Strong, natural leadership qualities coupled with exceptional communication skills.
