Overview
The Advanced Practice Provider (APP), either an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA), will provide telehealth clinical care and support for participants enrolled in the MOMitor maternal health program. MOMitor provides remote monitoring, screening, education, and care coordination for pregnant and postpartum patients, with the goal of identifying clinical concerns early and facilitating timely and appropriate care.
This position will primarily provide care through telehealth and remote patient monitoring. Working in collaboration with the MOMitor Medical Director and clinical team, the APP will provide evaluation and management of pregnant and postpartum participants with clinical concerns identified through MOMitor screening, remote monitoring, or communication with the care team. The APP will also serve as a clinical resource for the MOMitor nursing team and participate in the ongoing development and refinement of the program's clinical protocols and patient education initiatives.
Primary responsibilities include:
- Conduct telehealth visits with pregnant and postpartum MOMitor participants for evaluation, management, education, and follow-up of clinical concerns identified through the program.
- Review relevant clinical information and remote monitoring data and develop appropriate plans of care within established MOMitor protocols and the APP's scope of practice.
- Provide clinical consultation and support to MOMitor registered nurses, including responding to patient-related clinical questions and assisting with decisions regarding appropriate evaluation, treatment, follow-up, and escalation of care.
- Collaborate with the MOMitor Medical Director and clinical team to develop, review, and revise clinical protocols, algorithms, workflows, and patient education materials.
- Assist with ensuring that MOMitor clinical protocols reflect current evidence-based recommendations and standards of care for pregnancy and the postpartum period.
- Provide individual participant education regarding pregnancy, postpartum health, chronic medical conditions, warning signs, preventive care, and other relevant maternal health topics.
- Develop and/or facilitate virtual medical education sessions and group visits for MOMitor participants, which may include pregnancy and postpartum education groups.
- Coordinate with obstetric, primary care, behavioral health, and other specialty clinicians when additional evaluation or treatment is indicated.
- Facilitate appropriate escalation of care to outpatient clinics, urgent evaluation, emergency services, or other levels of care when clinically indicated.
- Document telehealth encounters and clinical recommendations in the electronic medical record in accordance with institutional policies and applicable regulatory requirements.
- Participate in interdisciplinary MOMitor team meetings and ongoing program development, quality improvement, and clinical workflow optimization.
- Perform other clinical duties related to MOMitor program operations as appropriate and within the APP's scope of practice.
Qualifications
Minimum Education and Experience Requirements:
Master's degree in Nursing and Physician Assistant. A minimum of one year of clinical experience in obstetrics is required. Experience should include the evaluation and management of pregnant and/or postpartum patients.
Credentialing:
Must meet requirements set forth by the Medical Staff Office to obtain and retain privileges to practice as an Advanced Practice Registered Nurse or Physician Assistant within UF Health Shands Hospital and/or the Shands UF Healthcare Network, as applicable.
Licensure/Certification/Registration:
Current and clear license as an Advanced Practice Registered Nurse (APRN) Nurse Physician Assistant (PA) in the State of Florida. BLS is required.
Preferred Qualifications
- Additional clinical experience in obstetrics, women's health, maternal health, or postpartum care.
- Experience with telehealth, remote patient monitoring, or population-based care.
- Ability to independently evaluate patients while working collaboratively within an interdisciplinary clinical team.
- Strong clinical judgment and ability to recognize conditions requiring escalation of care.
- Experience developing or implementing evidence-based clinical protocols.
- Interest in patient education and group-based pregnancy or postpartum care.
- Strong communication skills and comfort working closely with registered nurses, physicians, and other members of an interdisciplinary care team.
- Spanish language proficiency is preferred but not required.
