JOB TITLE: On-Demand Nurse Practitioner – Preventative Care JOB TYPE: Contract (1099) | WEEKEND/HOLIDAY SHIFTS ONLY LOCATION: Remote REPORTING TO : Head of Strategic Initiatives JOB SCOPE: We are seeking a dedicated and experienced Michigan-licensed Nurse Practitioner to join our team for an on-demand role. This position involves contacting patients with abnormal results from screening events and providing them with next steps and guidelines. The role is structured around scheduled events occurring on a weekend shift, with a minimum of two (2) Sundays per month. Routine shifts tend to be 6-8 hours. MAJOR JOB DUTIES: Contact patients with abnormal screening results to discuss findings and provide guidance on next steps. Deliver clear and concise information about the nature of the abnormalities and potential health implications. Develop and communicate personalized care plans, including necessary follow-up actions and preventive measures. Collaborate with a multidisciplinary team to ensure comprehensive patient care. Maintain accurate and timely documentation of patient interactions and care plans. QUALIFICATIONS: Active Nurse Practitioner license in the state of Michigan. Minimum of 2 years of clinical experience, preferably in a preventative care setting. Strong communication and interpersonal skills. Ability to deliver sensitive information with empathy and professionalism. Proficient in electronic health records (EHR) systems. Primary embraces diversity. This role will be paid at $40.00/hour (weekday), $50.00/hour (weekend), $60.00/hour (holiday) with the possibility of additional pay per case management call.
