Position Overview
We are seeking an organized, empathetic, and detail-oriented Patient Care Coordinator to join our remote clinical operations team. In this pivotal role, you will serve as a primary touchpoint for our patients, managing daily scheduling, handling incoming communications, and facilitating smooth care delivery across our service lines. This role is ideal for a proactive communicator who thrives in a fast-paced environment and is passionate about delivering exceptional patient experiences. Reporting to patient care leadership, the Patient Care Coordinator will focus on front-line communication, appointment coordination, and EHR documentation. This position requires someone who is dependable, highly organized, and comfortable navigating digital telehealth platforms while upholding high standards of patient care.

Primary Responsibilities
- Appointment & Patient Flow Coordination

Managing, scheduling, and rescheduling patient appointments accurately across clinical schedules.

Sending telehealth links, appointment details, reminders, and confirmations to ensure strong patient follow-through.

Managing no-shows and processing last-minute appointment changes to maximize schedule utilization.

- Patient Communication & Request Management

Handling incoming patient calls and messages in a professional, empathetic manner.

Processing and routing medication refill or request messages to the appropriate prescribing provider.

Directing general patient requests to the relevant department or provider while following up on outstanding items.

Communicating provider instructions to patients clearly and appropriately as directed by clinical staff.

- Clinical Operations Support & Intake

Assisting with patient intake, registration, and updating patient records.

Coordinating external referrals and required follow-up appointments to maintain continuity of care.

Documenting all patient communications accurately and thoroughly within the EMR.

Escalating urgent, high-priority, or clinical concerns immediately to the appropriate clinical staff.

Job Requirements
- Education:
- High School Diploma or equivalent required.
- Associate’s or Bachelor’s degree in Healthcare Administration, Communications, or a related field preferred.
- Experience and Qualifications:
- 0 to 2 years of experience in healthcare customer service, patient coordination, or medical front-office administration.
- Strong organizational skills, sharp attention to detail, and ability to multitask in a high-volume environment.
- Basic proficiency in Microsoft Office and comfort navigating EHR systems, telehealth platforms, or cloud-based tools.
- Clear, professional, and compassionate written and verbal English communication skills.
- Completion of a signed IRS Form W-8BEN prior to engagement start for non-U.S. remote workers to certify tax status.
- Schedule Requirements:
- Ability to work Eastern Time (ET) hours, 8:00 AM to 5:00 PM, Monday through Friday, including applicable company holidays.
- Technical Requirements:
- Reliable wired internet connection (100 Mbps minimum).
- Computer/laptop with Intel Core i5 or Ryzen 5 processor, 8GB RAM minimum, and 128GB free storage.
- Dual-screen setup preferred for enhanced productivity.
- Operating system: Windows 10 or macOS (latest version).
- Clear webcam (1080p or higher resolution) and microphone for communication.
- Secure, private workspace compliant with healthcare remote-work security requirements, prohibiting local downloads, printing, or screen capture of patient financial or health data.
