2. About the Opportunity:

The Patient Engagement Specialist is responsible for connecting prospective clients with Enhanced Care Management services. This role facilitates client enrollment by assessing needs, educating about available offerings, and linking clients with care teams. The position plays a critical part in improving client engagement and ensuring personalized care planning.


3. Responsibilities:

  • Engage with prospective clients through phone outreach and record interactions in the CRM system.
  • Make 60 to 100 outbound calls daily to potential clients.
  • Communicate client preferences and information to the care team for personalized care planning.
  • Assist clients in completing psychosocial assessments and developing actionable care plans.
  • Review medical charts to document client needs and recommend interventions.
  • Apply evidence-based practices such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
  • Participate in weekly team meetings to coordinate care delivery.


4. Requirements:

  • Ability to work Monday through Friday, 8:30 a.m. to 5:00 p.m. Pacific Time, with flexibility for evenings and weekends.
  • Active California Licensed Vocational Nurse (LVN) license, Medical Assistant certificate from a recognized group, or Bachelor’s degree in a related field.
  • Minimum of two years’ experience in telephonic outreach or enrollment in a high call volume setting.
  • Experience working with complex care clients and Enhanced Care Management populations.
  • Strong oral and written communication skills.
  • Positive interpersonal skills for effective client engagement.
  • Proficiency with technology including video conferencing, Google Workspace, Slack, and electronic health records; experience with Athena Health and Health Gorilla is a plus.
  • Prior remote work experience with a quiet, secure workspace and reliable internet.
  • Bilingual in English and Spanish is highly preferred.
  • Applicants must reside in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, or Washington.


5. Pay Range and Compensation Package:

  • The pay range and compensation package for this role will be determined based on the candidate’s experience, skills, and other relevant factors.
