Population Health Coordinator

• Analyze patient records across multiple health databases to pinpoint missing preventative care.
• Collaborate with providers to validate clinical care needs against electronic medical records.
• Lead patient outreach initiatives to proactively connect individuals with necessary medical services.
• Drive quality outcomes by ensuring our clinic maintains exceptional healthcare standards.

Licensure / Certification / Registration:

• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
  • Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.

Additional Preferences:

• Primary care experience
• Experience working with Athena
• Medical office billing and front office experience
• Must be located in the Nashville area
