Western Health Advantage (WHA) is seeking an organized and collaborative Population Health Program Specialist to help connect members with preventive care, wellness, health education, and chronic condition management programs.

In this role, you will lead the operational coordination of population health campaigns, member engagement initiatives, and incentive programs. You will collaborate with Marketing, Sales, Medical Management, external vendors, and other internal partners to deliver accurate, timely, and effective outreach that increases awareness and participation in WHA’s programs and services.

What You’ll Do

  • Serve as the primary operational contact for designated wellness and member engagement vendors, such as Active&Fit, Choose Healthy, and Healthy Recipes.

  • Coordinate population health outreach campaigns focused on preventive care, maternal health, chronic condition management, wellness, and member engagement.

  • Develop and maintain campaign calendars, communication schedules, project timelines, and related documentation.

  • Generate, validate, and manage member outreach lists for postcards, text messages, emails, newsletters, and other communications.

  • Coordinate the development, review, approval, distribution, and tracking of population health and wellness communications.

  • Partner with print vendors and internal teams to ensure campaign materials are produced and distributed on time.

  • Track and report campaign volume, language distribution, returned mail, participation, and other outreach metrics.

  • Coordinate the administration and distribution of member incentive programs and maintain accurate fulfillment records.

  • Maintain current program information, vendor resources, campaign materials, and member-facing content.

  • Support the development of educational materials, flyers, presentations, and communications related to population health programs and wellness services.

  • Review communications for accuracy and alignment with approved clinical program information.

  • Serve as the primary Population Health Management liaison to Marketing and Sales for program materials and communications.

  • Maintain resources that support Sales, employer groups, brokers, Member Services, Medical Management, and other internal stakeholders.

  • Monitor campaign performance and identify opportunities to improve member awareness, participation, and program utilization.

  • Recommend improvements that address operational, communication, or member engagement gaps.

  • Maintain compliance with applicable state and federal regulations, NCQA accreditation standards, contractual requirements, organizational policies, and HIPAA requirements.

  • Complete special projects and perform other duties as assigned.

What You’ll Bring

  • A bachelor’s degree in Public Health, Health Education, Nutrition, Healthcare Administration, Communications, or a related field is preferred.

  • Certification as a Health Education Specialist, Wellness Coach, or Medical Assistant is preferred.

  • Medical Assistant certification combined with relevant experience in health education, member engagement, outreach, population health, wellness programs, or healthcare communications may substitute for the preferred bachelor’s degree.

  • Two to four years of experience in healthcare, population health, wellness programs, health promotion, member engagement, healthcare communications, or a related field.

  • Strong organizational and project coordination skills, with the ability to manage multiple campaigns, priorities, timelines, and deliverables.

  • Excellent written and verbal communication skills, including experience supporting member-facing communications and educational materials.

  • The ability to collaborate effectively with Marketing, Sales, Medical Management, external vendors, and other stakeholders.

  • Working knowledge of healthcare terminology and population health concepts.

  • Experience generating, validating, and managing outreach lists, campaign targeting, communication distribution, and related reporting.

  • The ability to analyze outreach and engagement results and recommend ways to improve campaign effectiveness and member participation.

  • Proficiency with Microsoft Excel, Word, Outlook, PowerPoint, and Teams.

  • Experience with reporting platforms, healthcare applications, business intelligence tools, or similar systems—such as Cognos, Facets, or BI dashboards—is preferred.

  • The ability to speak, read, write, and understand the primary language used in the workplace.
