Practice Consultant – Social Worker | 11267 | permanent contract | 37 hours per week | £45,268 to £49,818 per annum | (Grade K) | Norfolk & Norwich University Hospital, Norwich

We offer a £2000 welcome payment for qualified and experienced Practice Consultants new to Norfolk County Council. Amount to be repaid in full should you leave within two years of employment.

For experienced Social Workers new to Norfolk County Council, we promise to try and match or better your existing salary, providing it's in the spectrum of the grade advertised.

An exciting opportunity has arisen to join the Central Norfolk Home First Hub. We are recruiting for a permanent Practice Consultant to join our well-established team. The service supports hospital discharge across Central Norfolk and delivers adult safeguarding responsibilities in line with the Care Act 2014.

The team promotes a Home First approach, aligned with national NHS guidance, ensuring people receive the right support to make a safe and successful transition from hospital back to their home and community.

This is a fast-paced team based at the Norfolk and Norwich University Hospital. Regular office attendance is required to support integrated working and leadership across health and social care services. As a Practice Consultant, you will provide leadership and supervision to Social Workers and Assistant Practitioners, with a strong focus on professional development, wellbeing, and high-quality practice.

You will lead and support complex casework, chair meetings, and facilitate multi-agency discussions to ensure effective decision-making and positive outcomes for individuals and their families.

A key aspect of the role is providing oversight of adult safeguarding referrals. You will be responsible for triaging safeguarding concerns, managing risk, and ensuring appropriate and timely decision-making. Working closely with partner agencies, you will support the development of safe discharge plans that minimise the risk of harm and promote individuals' independence and wellbeing.
