How it works

• We partner with local personal care homes and assisted living residences, and our experienced NPs care for their residents through your practice.
• You serve as collaborating physician for our NPs, by phone, with no on-site supervision.
• Our team and AI platform run your Medicare care management programs (CCM, APCM and PCM), plus scheduling, documentation and billing.
• You stay the owner. Everything bills through your clinic, with no new staff and no new overhead.

What's in it for you

You're a fit if you

• Are an MD/DO who owns a Medicare-certified primary care clinic (1–2 locations) in Montgomery, Bucks, Chester or Delaware County
• Have Medicare set up as payee and hold an active Pennsylvania license and DEA registration
• Are board certified or eligible in Internal Medicine, Family Medicine or Geriatrics

Work location: On the road: visits at local senior living communities

Job type: Part-time, Contract (W-2 or 1099)

Pay: $1,275–$1,530 per day (paid $85 per patient visit; about 15–18 visits a day)

Schedule: Monday to Thursday, flexible; a couple of visit days a month or more
