Elevate Health is one of nine Accountable Communities of Health (ACHs) in Washington State, a successful non-profit collaborative in Pierce County, and dedicated to achieving health equity through collective action (learn more at elevatehealth.org). We have a rare opening for a Program Specialist
in our (hybrid) Tacoma office. If you are looking for an excellent career opportunity at a successful, mission- and values-driven organization, with an amazing team, and a hybrid work schedule, this is a unique opportunity!
The Program Specialist will primarily assist the programs team by ensuring referrals and case assignments are entered, processed, and proceeded through care coordination workflows as specified. In times of high referral numbers, the Program Specialist may provide direct client support to community members and clients in the form of short-term resource navigation and resource brokering. On an as-needed basis, the Program Specialist will also help individuals and families understand, access, and enroll in public health and income support programs that improve health and stability. Working in community settings, the Program Specialist provides culturally responsive, person-centered assistance with eligibility screening, applications, renewals, and education for programs including Medicaid, Medicare, Apple Health, and other state and federal public benefit programs. The Program Specialist serves as a trusted resource, building relationships with community members and partner organizations to reduce barriers to healthcare access and promote long-term benefit retention.
Here's what you'll be doing:

- Work in collaboration with all departments at Elevate Health and with diverse sectors, community members, and other stakeholders to continually improve work processes and enhance service provider and client experience.
- Work to improve Community Care Hub processes, communication, and client care as it relates to health-related social needs (HRSN).
- Support the operations of the department and Elevate Health as necessary
Other Duties as Assigned
This position may be assigned additional duties and responsibilities beyond those listed above. While these activities may be required periodically, they are not core functions of the role.

- Maintain accurate records in required databases and case management systems.
- Track outreach activities, enrollments, and performance metrics.

- Experience working with diverse populations and individuals experiencing barriers to healthcare or public benefits.
- One to two (1-2) years of experience in processing referrals within program requirements using client management system or similar technology.
- Any combination of education, professional, volunteer and lived experience that provides the necessary knowledge, skills, and abilities to perform the duties and responsibilities.
- Strong communication, interviewing, and problem-solving skills.
- Proficiency with Microsoft Office and web-based applications
