Position Description
Surrey Place is seeking a Registered Nurse in Urgent Response Services. The successful applicant will report to the Manager, Complex Community Teams. Clients receiving service in this program have a diagnosis of Autism and other Developmental Disorders and exhibit behavioural, communication, and socialization challenges requiring an urgent response for up to 12 weeks. Services will include direct assessment and implementation of treatments with clients and a focus on building parent/mediator capacity to effectively support their child/youth.
This position will play a critical role in screening to determine clients who may have health related needs that would benefit from more extensive medical/health input. This position will also be a critical member of some family service teams for URS clients, either as the lead for medical assessment/intervention or assisting with the referral and liaison with current or new medical professionals who become involved in treatment and care.
Main Responsibilities
- Collaborate with URS interdisciplinary team to determine a client’s suitability for Surrey Place medical services, prioritize and triage clients for waiting list management.
- Conduct comprehensive health assessments of infants, children and youth with developmental disabilities including ASD. Assessments include a holistic health history and physical examination.
- Formulate, implement, and evaluate plan of care, involving clients and/or care givers for health needs appropriate to RN scope of practice and specifically focused on developmental disabilities using evidenced-based care, and document plan of care as part of the URS family service plan.
- Contribute to the management of episodic and stable chronic conditions commonly seen in primary health care, consulting with Surrey Place or external physicians, Nurse Practitioner, and/or Clinical Nurse Specialist.
- Work with clients, families, internal and external inter-professional teams to navigate the health care system, identify and access health and other resources in their communities.
- Act as the primary contact person for families and their child’s medical team for clients requiring pharmacologic management support.
- Collaborate with interdisciplinary team on client’s documents, including writing reports (intake, formulation, intervention and discharge), and communicating with professionals and caregivers.
- Provide nursing consultation and education to the client’s team, including family, service providers, community pediatricians and family physicians on the impact of various medical conditions and medication on a child’s behaviour and functioning.
Qualifications
- Current registration as a Registered Nurse in the General Class with the College of Nurses of Ontario, and certificate of continuing competence.
- Baccalaureate degree in nursing from a recognized university or equivalent education and experience, with willingness to pursue higher education, such as a master’s degree.
- Three years of nursing practice experience in a community setting or a combination of community experience and public health nursing is essential. Relevant hospital nursing is an asset.
- Experience working with individuals with developmental disabilities and mental health issues is an asset.
- Experience with case management an asset
- Thorough current nursing knowledge and proficiency in conducting a comprehensive health history, physical assessment, and construction of a nursing management plan.
- Demonstrated ability to work effectively in a multi-disciplinary environment.
- Demonstrated commitment to and knowledge of community-based health care.
- Experience working in psychopharmacology.
- Knowledge of pediatric developmental and medical issues.
- Knowledge of community resources and supports.
- Good organization, communication and interpersonal skills, with a demonstrated ability to work well with others.
- Proficiency with Microsoft Office (MS Word, PowerPoint, Excel, and Outlook).
- The successful candidate must provide an acceptable Police Records Check (Vulnerable Sector Screening).
- Fluency in French is considered an asset.
