Role Description This full-time Registered Nurse role is a hybrid position based in Nundah, QLD, with a mix of in-home client visits and some work from the office and home for care coordination and documentation.


The Registered Nurse will provide comprehensive clinical assessments, develop and review care plans, administer medications, perform wound care and other nursing procedures, and monitor changes in client health status.


The role involves collaborating with clients, families, caregivers, and allied health professionals to ensure high-quality, person-centered care that aligns with NDIS and aged care standards.


Daily responsibilities include maintaining accurate clinical documentation, contributing to risk assessments, supporting health education for clients and families, and participating in case conferences and service planning.


The Registered nurse will also help ensure compliance with relevant clinical guidelines, organizational policies, and regulatory requirements while promoting safety, dignity, and independence for clients in their homes.


Qualifications

  • Current registration as a Registered Nurse (RN) with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Demonstrated clinical skills in community or home care nursing, including assessment, care planning, wound care, medication administration, and chronic disease management.
  • Experience or strong interest in aged care, disability support, or NDIS-funded services, with an understanding of relevant standards and regulations.
  • Strong communication and interpersonal skills, including the ability to build trust, advocate for clients, and collaborate effectively with multidisciplinary teams.
  • Proficiency in electronic clinical documentation, basic digital literacy, and the ability to manage scheduling and reporting using home care systems.
  • Ability to work autonomously, prioritize competing demands, and adapt to changing client needs in a community setting.
  • Commitment to person-centered care, cultural safety, and non-discriminatory practices that respect diverse backgrounds and identities.
  • Valid driver’s license, reliable transport for community visits, and necessary clearances (e.g., National Police Check, NDIS Worker Screening) or willingness to obtain them.
  • Relevant postgraduate training or experience in community nursing, gerontology, or disability care is desirable but not essential.
