- Location: Indianapolis, IN
- Facility: Ascension Medical Group Clinic
- Department: Heart Care
- Schedule: Monday - Friday 7am-5:30pm | 10 hours 4 days per week | partially remote
- Implement and monitor patient care plans by expertly assessing and evaluating symptom trends over the phone, updating the healthcare team, and providing immediate clinical guidance.
- Execute physician and nursing orders with precision, acting as the vital link between the Cardiology team and the patient to ensure treatment plans are followed accurately at home.
- Coordinate seamless care transitions by managing discharge planning and providing specialized education on heart health, medication adherence, and post-procedural recovery.
- Lead clinical triage by utilizing advanced cardiac assessment skills to determine the safest and most effective level of care for patients experiencing cardiovascular symptoms.
- Required Credential(s):
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
- Registered Nurse obtained prior to hire date or job transfer date.
- Preferred Credential(s):
- Advanced Life Support. American Heart Association or American Red Cross accepted.
- Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
