To learn more about us, please visit www.thearcofpgc.org. Position Summary: The Arc Prince George's County is seeking a full-time Delegating Registered Nurse (RN) - Case Manager . Reporting to the Director of Nursing, the delegating RN provides nursing support in the form of case management for assigned individuals in The Arc’s Employment, Day, and Community Supports Services as noted above, participates actively as a member of individuals’ IP teams as needed, and teaches and supervises staff in the performance of delegated nursing tasks. Here’s What You’ll Be Doing Day-to-Day: Initial Nursing Assessments, with updates as needed, for all Employment Services, Day, and Community Supports Services participants assigned; includes Self-Medication Assessment and Choking/Aspiration Pneumonia Risk Assessment Development of Nursing Care Plans and updates as needed for all individuals requiring delegated nursing functions Nursing reviews of health status and delegated care (i.e., “45-day reviews”) for all individuals requiring delegated nursing functions; reviews/assessments may be required more often according to individuals’ needs Teaching and/or written verification of teaching and competency of staff in the performance of delegated nursing functions Regular review (at least annually) and documentation of staff competency in the performance of delegated nursing functions On-call (taking phone calls) nursing coverage for all assigned sites and individuals Sunday evening through Friday evening for Residential, Monday-Friday business hours for assigned Day Services program(s) Participation in weekend on-call rotation for Residential Services from Friday evening to Sunday evening, shared by all Day and Employment Services delegating RNs. Provide call coverage by accepting calls from staff and provide direction as needed when those staff are not able to reach their assigned Delegating Nurse. Appropriate, prompt response to emergencies and other health issues reported by staff Follow-up of individuals’ health status in the event of significant changes, especially including discharge from hospitals or other care facilities, to determine need for additional staff training or changes in Nursing Care Plan Written recommendations/guidelines for Self-Medication, shared with the team for any individuals with self-medication as part of IP goals Liaison with home caregivers, families, and Residential Services provider agencies regarding individuals’ health care, for individuals in Day Services as applicable. Share teaching of MTTP and Two-Year Clinical Medication Update classes on an equitable basis (may vary according to MTTP training personnel and schedule). Teaching, supervision, and support of LPNs, CNAs, Certified Medication Technicians (CMTs) and other Arc staff involved in individuals’ health care Attendance at staff meetings in all applicable programs when required for staff teaching or other nursing-related issues Communication with other Delegating RNs (in the Arc, and outside the Arc) regarding health care issues for assigned shared individuals Attendance at IP meetings where health issues are a significant/critical part of the IP process Consistent, thorough, and timely documentation of all nursing assessments, visits, teaching, and interventions Ensure compliance with all DDA and Maryland Board of Nursing policies and directives Communication with Residential Program Directors and Coordinators, day services Program Directors and Coordinators, Employment Services Program Directors and Administrators, and Community Supports Coordinators, regarding health care issues of participants when applicable. Teaching and counsel for individuals where applicable to help them achieve and maintain optimum health Attendance at a minimum of two DDA Nurses’ Quarterly meetings annually Maintain HRST Clinical Reviewer status. Participate in all DDA required training. Must be able to perform all essential duties with or within a reasonable accommodation. You Are Our Ideal Candidate If You: Active RN Licensure in the State of Maryland, preferably with Bachelor of Science degree in Nursing from an accredited school of nursing. Two (2) years of clinical nursing experience, preferably with one (1) year of experience working with individuals with developmental disabilities, and preferably with one (1) year of supervisory experience of unlicensed assistive personnel. Must have a thorough knowledge of the Maryland Nurse Practice Act regarding delegation of nursing functions. Able to use basic Microsoft Office applications, including Word, Excel, and Outlook. Strong oral and written communication skills. Strong interpersonal skills. Effective, professional, respectful oral and written communication with all levels of staff, with individuals, with families, and with all other persons involved in participants’ care Positive, “can-do” attitude toward challenges and problems Commitment to the person-centered process for developing and implementing services and supports.
