Remote LPN/LVN Care Navigator, Care Management
New York state license required. Hiring in multiple states. Compact licensure is preferable but not a prerequisite depending on state of residence.
Pay, schedule, and benefits
Pay: $21 to $29 per hour, based on experience and state of residence, plus a quarterly bonus of up to $1,500.
Schedule: Full-time, 40 hours per week, Monday to Friday, in Eastern Time. Week 1 of training runs 9:00 am to 5:30 pm Eastern. After that, you join your team and your work schedule is set.
100% remote work from home, with paid training on our remote patient monitoring platform and reimbursement of additional state licenses we ask you to obtain.
Benefits: Majority employer-paid medical, dental, and vision coverage, paid time off, parental leave, and career growth opportunities, including a path to Nurse Supervisor.
What you will do
- Build relationships with your patients that help them reach their goals and manage conditions such as hypertension and diabetes, in line with provider-approved care plans.
- Support patients enrolled in care management and remote patient monitoring programs through outbound and inbound calls, and teach them to manage their health.
- Review readings such as blood pressure, blood glucose, weight, and pulse oximetry, and follow approved partner workflows.
- Document every patient interaction accurately in the EHR and escalate concerns to the care team.
What you bring
- An active New York state LPN/LVN license. A multistate (compact) license is preferred. If you hold a single-state license, you may still qualify depending on your state of residence, and you will need to apply for your compact license before your start date.
- 1+ year of experience in primary care, cardiology, internal medicine, home care, or care management. Remote patient monitoring experience is a plus.
- Comfort with EHRs, care management software, and tools such as Zoom and Slack.
- A HIPAA-compliant, distraction-free workspace and reliable internet at your own cost (100 Mbps download, 10 Mbps upload). HealthSnap provides your equipment.
What the role is like
Manage your assigned panel of 150 to 300 patients, depending on the program mix. This includes outbound calls and responding to incoming calls, SMS, and emails. Outbound call volume ranges from 20 to 40 calls per day, depending on answer rates.
Training starts with one week with Learning and Development and one week with the Coaching Team. Then comes six weeks in the ramping phase, in which your caseload grows to the full assignment.
How to apply
The application takes 5 to 10 minutes. We typically review new applications within 1 to 2 business days. HealthSnap is an equal-opportunity employer, and all qualified applicants will receive consideration without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability, or protected veteran status.
