Apply Description Receive, review, and respond promptly to assigned intake referrals Conduct biopsychosocial assessments and support diagnostic formulation under supervision Develop and implement individualized treatment plans with guidance from a Clinical Supervisor Provide direct clinical services, including individual, group, family, and/or couples therapy Maintain an active caseload appropriate to level of experience, with ongoing clinical oversight Monitor patient progress and adjust treatment approaches in collaboration with a supervisor Support discharge and aftercare planning to ensure continuity of care Provide case management services and coordinate referrals to external resources when needed Maintain accurate, timely, and compliant documentation within the Electronic Health Record (EHR) Ensure coordination and integration of services documented across providers and disciplines Collaborate with prescribers and other members of the care team to support patient outcomes Participate in individual and group supervision in accordance with licensure requirements Attend treatment team meetings, staff meetings, and clinical trainings Comply with all local, state, and federal confidentiality and regulatory requirements Deliver culturally responsive, patient-centered care aligned with PAX Health values Abide by PAX Health code of ethics and model values Perform other duties as needed within the scope of the role Requirements Master’s degree in Social Work, Counseling, Psychology, or a related field Active and unrestricted provisional license (e.g., LSW, LAC, or equivalent) Working toward independent clinical licensure under appropriate supervision Foundational clinical skills in assessment, treatment planning, and therapeutic interventions Interest in developing skills in evidence-based modalities such as CBT, DBT, or similar approaches Experience working with diverse populations and a range of mental health conditions preferred Strong communication and interpersonal skills Strong organizational and documentation skills Proficiency with EHR systems Ability to work collaboratively within a multidisciplinary team Commitment to professional growth, supervision, and ongoing learning Commitment to ethical practice and patient-centered care LCADC or CADC is a plus Equal Opportunity Employer (EOE) Statement for PAX Health PAX Health is an equal opportunity employer and prohibits discrimination and harassment of any kind.
