Overview:

The CDC Foundation is seeking a Rural Health Transformation Community Health Worker (CHW) Support and Partnership Coordinator to support the Kentucky Rural Health Transformation Program (RHTP). The Kentucky RHT is a multi-year, statewide initiative to modernize rural health systems and improve access, quality and sustainability for rural Kentuckians. The program operates across five interdependent initiatives — Rural Community Hubs, PoWER (perinatal health), Rapid Response to Recovery (behavioral health and crisis care), Rooted In Health (integrated/oral health), and Crisis To Care (community trauma care and EMS) — coordinated by a central program office.

This role provides statewide technical assistance and implementation support to organizations participating in the Rural Health Transformation Program (RHTP). This position partners with pilot sites, healthcare organizations, public health agencies and community partners to support the successful integration of Community Health Workers into clinical and community settings through evidence-based implementation strategies, workflow development, quality improvement and workforce development.

The Coordinator serves as a subject matter expert on Kentucky's CHW model, providing coaching, technical assistance, and program support while fostering collaboration, promoting adherence to state standards, and advancing sustainable CHW programs that improve health outcomes across Kentucky's rural communities. This role will be embedded within the Kentucky Department for Public Health's Kentucky Office of Community Health Workers, expanding the office's implementation capacity while serving as a key liaison between the Kentucky Office of Community Health Workers and RHTP leadership to advance statewide RHTP Community Health Worker initiatives.

Required Qualifications:

· 5+ years of experience as a Community Health Worker, social worker, public health professional, or in a related field required. A bachelor's degree in public health, social work, or a related field is preferred; however, equivalent experience may substitute for formal education.

· Demonstrated ability to effectively communicate with diverse audiences, facilitate trainings and presentations, solve problems collaboratively and drive successful implementation of community-based programs and initiatives.

· Strong understanding of the CHW certification, continuing education and certification renewal requirements as outlined by the Kentucky Office of Community Health Workers

· Demonstrated ability to facilitate productive conversations, navigate conflict and build consensus among diverse stakeholders while maintaining positive, collaborative relationships.

Preferred Qualifications:

· Active Kentucky CHW Certification issued by the Kentucky Office of Community Health Workers.

· Experience working with rural communities and an understanding of the unique health, workforce and social challenges facing rural populations preferred.

· Experience with Community Health Worker workflows, needs assessments, documentation, best practices and sustainable service delivery models, including Community Health Worker billing, preferred.

· Experience supporting program implementation, quality improvement, program evaluation, data collection or performance monitoring preferred. Examples may include collecting participant outcomes, documenting client encounters, tracking program metrics, or using data to inform continuous improvement efforts.

Responsibilities:

Program Implementation & Technical Assistance

· Provide technical assistance to organizations implementing Rural Health Transformation Program (RHTP) Community Health Worker (CHW) initiatives, including program design, workflow development, implementation planning and CHW onboarding.

· Support the successful integration of RHTP-funded CHWs into clinical and public health settings by promoting evidence-based practices, sustainable workflows and effective interdisciplinary collaboration.

· Deliver ongoing coaching, technical assistance and implementation support to RHTP-funded CHWs, supervisors and pilot site leadership to promote workforce success, role clarity, job satisfaction and retention.

· Conduct regular site visits to assess implementation progress, identify opportunities for quality improvement and provide hands-on support to pilot site teams.

· Promote fidelity to the Kentucky Community Health Worker scope of practice by ensuring pilot site CHWs operate in accordance with Kentucky Revised Statute and Kentucky Administrative Regulations, program standards and established best practices.

Program Development & Quality Improvement

· Develop, implement and continuously refine CHW workflows, implementation strategies and technical assistance resources to support consistent, high-quality program delivery.

· Collect, interpret and use program data, stakeholder feedback and implementation findings to inform continuous quality improvement and program refinement.

· Develop technical assistance resources, tools and engagement strategies to support effective utilization of kynect resources and other care coordination platforms to support closed loop referrals.

Partnerships, Collaboration & Workforce Development

· Collaborate with the Office of Community Health Workers, RHT leads and other Department for Public Health and Cabinet for Health and Family Services programs to strengthen CHW certification, continuing education, workforce development, professional growth, data collection, evaluation and other initiatives aligned with RHTP goals.

· Facilitate collaboration and serve as a liaison among RHT pilot site CHWs and community partners to strengthen communication, resolve implementation challenges and advance shared program goals.

· Build and maintain collaborative relationships with healthcare providers, community organizations, educational institutions and other stakeholders to advance CHW workforce strategies and support sustainable program implementation.

Education, Outreach & Representation

· Actively engage in Kentucky Community Health Worker education, networking and professional development activities to strengthen statewide partnerships, share best practices and stay informed of emerging trends, policies and workforce initiatives.
