About the Role

Family Pillars Homecare is seeking an organized, clinically skilled Registered Nurse to manage and coordinate home care services for patients throughout Southwest Florida.

We are recruiting candidates based in Naples, Lehigh Acres, or Fort Myers; however, this position’s caseload will include patients across the Southwest Florida region—not just those three cities.

This primarily remote position requires in-person patient visits as needed. You will coordinate managed care, workers’ compensation, and private-pay cases, serving as a key clinical point of contact for patients, families, caregivers, therapy providers, payers, and adjusters.
Our ideal candidate combines strong clinical judgment with excellent communication, attention to detail, and the ability to navigate different payer requirements while keeping patient care organized and responsive.

Key Responsibilities
  • Manage a caseload of managed care, workers’ compensation, and private-pay patients throughout Southwest Florida.
  • Review and update individualized care plans in collaboration with patients, families, caregivers, providers, and other care team members.
  • Conduct in-home assessments and patient visits as needed based on clinical needs and case requirements.
  • Monitor changes in patient condition, identify concerns, and coordinate timely follow-up.
  • Coordinate services within approved authorizations, including monitoring authorized hours, visits, service dates, and requests for additional care.
  • Communicate with managed care representatives, workers’ compensation adjusters, external case managers, and treating providers regarding patient progress, care needs, and service coordination.
  • Coordinate private-pay services with patients and families, clearly communicating the care plan, service expectations, and changes in care needs.
  • Maintain accurate, timely clinical documentation and case updates that meet agency standards and applicable payer requirements.
  • Collaborate with scheduling, HR, and clinical leadership to address staffing needs, communicate case-specific instructions, and support continuity of care.
  • Assist with onboarding and orientation of field staff assigned to your cases.
  • Identify and address barriers to care, service gaps, and patient or family concerns.
  • Assist in daily Quality Assurance of notes and files, not limited to SOC's, evaluations and all clinical visits including Therapy.
Required Qualifications and Skills
  • Active Florida Registered Nurse (RN) license.
  • Strong clinical assessment, care planning, and patient monitoring skills.
  • Ability to coordinate care across managed care, workers’ compensation, and private-pay cases, recognizing that each has different communication, authorization, and documentation needs.
  • Understanding of service authorizations and the ability to track approved services, identify upcoming expirations, and communicate requests for changes or extensions.
  • Strong professional communication skills when working with patients, families, caregivers, providers, payers, and adjusters.
  • Ability to explain care plans and service expectations clearly, manage concerns professionally, and maintain appropriate confidentiality.
  • Excellent organization, documentation, time management, and follow-through.
  • Comfort using electronic documentation systems and remote communication tools.
  • Ability to work independently, prioritize competing needs, and manage a geographically dispersed caseload.
  • Reliable transportation and willingness to travel throughout Southwest Florida for patient visits as needed.
Preferred Experience
  • Home care or home health case management.
  • Managed care coordination, including payer communication and authorization tracking.
  • Workers’ compensation case coordination, including communication with adjusters, external case managers, and treating providers.
  • Private-pay care coordination and direct communication with patients and families regarding service needs and expectations.
  • Coordination of nursing, caregiver, and therapy services.
Why Join Family Pillars Homecare?
  • Hourly pay of $38–$40.
  • Primarily remote work with patient visits as needed.
  • Supportive, responsive clinical leadership.
  • A collaborative team focused on coordinated, patient-centered care.
  • An opportunity to help grow and strengthen home care services throughout Southwest Florida.
