POSITION SUMMARY

The Access Mental Health (AMH) Senior Clinician provides clinical consultation primarily through telephonic support for school-based health center (SBHC) programs in Wheeler’s AMH Hub region. This may include facilitating referrals to the team’s Child Psychiatrist for a face-to-face evaluation or consultation, supporting SBHC staff with behavior strategies and linking appropriate services for students. The Senior Clinician will do outreach and engagement with SBHC sites to promote services and increase visibility of services with a focus on SBHCs in CT’s rural communities. The Senior Clinician will foster positive working relationships with SBHC staff, families and outside key stakeholders and utilize a strength based-solution focused model of care.

Eligible for a $3,000 sign on bonus! Half at hire and half at 1 year of service!

SALARY $62,000

EDUCATION AND EXPERIENCE/QUALIFICATIONS

Master's or Doctoral degree in Social Work, Marriage and Family Therapy, or Counseling plus a minimum of 2 years of progressively responsible clinical experience is required. Some travel is required to provide community outreach services.

Must have a driver's license and access to reliable transportation.

Licensure Requirements for Clinical Positions

Credential(s) Required: Licensed in the State of Connecticut (LCSW, LPC, LMFT, Psychologist)

LOCATION

Position is approximately 80% remote, and also requires community outreach. Office location is in Plainville, CT.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Participation in team activities includes but not limited to client screening, assessment and referrals; coordination of community based trainings and cross-coverage as needed.

Provide excellent customer service to key stakeholders including but not limited to SBHC sites and staff, physician practices, other service providers, families, and program stakeholders.

Conducts program specific screening of incoming AMH calls and triages calls based on needs to appropriate routes of consultation within the AMH service scope.

Demonstrates knowledge of internal and external levels of care, referral processes and resources. Makes clinically appropriate referrals for identified service needs in a timely manner.

Communicates in a positive, effective manner with SBHC site staff, client, family, and all internal and external providers. Attends any required meetings including outreach events and opportunities to promote the visibility and accessibility of the AMH SBHC consultation service.

Meets direct service productivity goals as defined by program. Demonstrates knowledge of program/department/agency goals and target outcomes and adheres to practice standards and to data collection and reporting protocols.

Documents all client relation information in compliance with agency policy.

Attends and is prepared for scheduled supervision. Seeks additional supervision or consultation as needed and follows through with supervisory directives.

Provides service in the office and/or community settings outside of the office (e.g., homes, schools, hospitals, institutions, other community location) as defined by the program.

Proficient with electronic health records.
