Show job description

We are currently looking for a Senior FHIR Data Platform Engineer for a 100% remote position supporting a large federal government healthcare modernization project. This role will optimize and maintain the HAPI FHIR data platform, with a strong focus on PostgreSQL performance, FHIR resource indexing, data quality, Master Data Management (MDM), and healthcare data reporting. The engineer will also develop FHIR-based quality measurement services, support HEDIS reporting, and work with large-scale healthcare datasets to ensure reliable, accurate, and performant data exchange.

The role operates in a highly collaborative environment, partnering with cloud platform engineers, data engineers, architects, and healthcare interoperability teams to optimize database performance, support reporting initiatives, maintain platform availability, and deliver production-ready solutions. The engineer will work extensively with HAPI FHIR, Java, Spring Boot, PostgreSQL, OMOP, and related data technologies.

The candidate must reside within the continental US.

Responsibilities

  • Optimize the HAPI FHIR JPA persistence layer within PostgreSQL, including search parameters, custom indexes, partitioning strategies, reindexing, resource expunge and history policies, Bulk Data export performance, and asynchronous MDM link tables.
  • Design and execute FHIR data quality checks, including US Core STU 9 conformance, duplicate and orphaned reference detection, and source-event precedence validation.
  • Develop and maintain measure-reporting services for digital quality measurement engines and HEDIS baselines, including FHIR-based measure evaluation, facility and statewide reporting, administrative-effort baselines, and DHHS monthly service report exports.
  • Query OMOP Common Data Model and Apache Iceberg data marts for reporting purposes and collaborate with data engineers on the transformation of published healthcare data into OMOP when FHIR-side changes affect downstream datasets.
  • Produce capacity planning and performance evidence supporting 99.9% availability, 15-minute Recovery Point Objective (RPO), and 4-hour Recovery Time Objective (RTO) requirements for the FHIR platform.
  • Support cloud platform engineers with Aurora PostgreSQL sizing, read-replica configurations, backup verification, and database performance optimization while delivering migration scripts, runbooks, dashboards, and test suites into designated repositories.
  • Take on additional tasks and responsibilities as needed to support team objectives and ensure the success of the project.

Requirements

  • Bachelor's degree in Computer Science, Information Technology, Data Engineering, or a related field and 7+ years of professional PostgreSQL experience in production environments.
  • Strong hands-on experience with PostgreSQL query planning, index design, partitioning, and performance troubleshooting across multi-terabyte databases.
  • 3+ years of hands-on experience with HAPI FHIR JPA server internals, including resource tables, search-parameter indexing, Bulk Data, MDM, FHIR R4, and US Core.
  • Working knowledge of OMOP Common Data Model (CDM) v5.4, including concept, vocabulary, and clinical-event tables, with experience writing analytical SQL queries.
  • Experience developing healthcare quality reporting solutions using FHIR-based measures, CQL, FHIR Measure/MeasureReport, digital quality measures (dQM), HEDIS, or equivalent clinical quality reporting standards.
  • Proficiency in Java 21, Spring Boot, Flyway, and Git, with the ability to extend HAPI FHIR interceptors and measure-reporting services while following secure development practices in HIPAA-regulated environments.

Additional Qualifications

  • Experience with Amazon Aurora PostgreSQL, Apache Iceberg, Apache Spark, Kafka Streams, OpenSearch, and Grafana.
  • Experience supporting Medicaid programs, state Health Information Exchanges (HIEs), or VA/federal healthcare data modernization initiatives.
  • Experience working with healthcare data extracts from Epic, Meditech, Oracle Health, or athenahealth.
  • Experience with OHDSI tools such as Achilles or Data Quality Dashboard (DQD) and healthcare data quality validation.
  • Experience with high-availability database architectures, disaster recovery planning, backup verification, and production performance monitoring.

Security Clearance Requirements

  • U.S. Citizenship is required.
  • All selected candidates must successfully complete a background check.
  • Ability to obtain and maintain a Government/Public Trust clearance, including required fingerprinting, when required for the position.

Benefits

  • Medical/Dental/Vision
