- Support people through significant life challenges, helping them make informed choices and achieve their desired outcomes.
- Work collaboratively with health colleagues, housing providers, voluntary organisations and community partners.
- Maintain accurate case recordings, assessments and reports to support good decision-making and effective practice.
- Participate in duty arrangements, team meetings, peer support and service improvement initiatives.
What You'll Bring
- A recognised Social Work qualification and registration with Social Work England.
- Experience of working within Adult Social Care or a related social care setting.
- Knowledge of relevant legislation, safeguarding responsibilities and strengths-based practice.
- Excellent assessment, communication and relationship-building skills.
- The ability to work independently whilst contributing positively to a team environment.
- Resilience, professionalism and the ability to manage competing demands effectively.
