Backed by Y Combinator, Foundation Capital, Soma Capital, and other amazing investors, Cair Health has raised over $3M to build the agent-operated revenue cycle management platform for healthcare. We're at $1M+ ARR, profitable, and grew 21x last year. Our customers include clinics, MSOs, digital health companies, EHRs, FQHCs, billing companies, and more.
Day to day, we work with operations leaders, heads of RCM, billers, and sometimes engineers. But the product itself is deeply technical, and every account eventually needs someone who can configure the system, troubleshoot integrations, or work directly with a customer's engineering team on our APIs and their data flow.
About the role
We're hiring a Software Engineer to build the core agent-operated platform underneath everything Cair does. A lot of what customers need starts as a one-off build for a specific account; your job is to take what works and turn it into a product: generalize it, harden it, and make sure the platform scales as we add customers.
As one of our first engineering hires, you'll help shape the architecture, the agent framework, and the engineering practices that every future engineer inherits.
What you'll do
Own core parts of the agent-operated RCM platform, from architecture through production
Take integrations and workflows built for individual customers and generalize them into the core product
Design and build the agent framework and infrastructure that the rest of the team builds on top of
Ship fast and iterate directly based on what customers and the field team are running into
Debug and fix issues across the stack, from data pipelines to the agents themselves
Set technical standards, code review practices, and engineering processes as an early hire
Stay close to what customers actually need, not just what's on the roadmap
Required
You'll need a CS degree or equivalent hands-on experience and 3+ years in a technical, customer-facing role (solutions engineering, forward deployed engineering, implementation, or technical customer success).
Deep software engineering fundamentals: system design, data modeling, testing, and the judgment to know when to generalize something versus keeping it simple
Experience writing Python (or similar), SQL, and React on the front end
Working knowledge of REST APIs
Experience with AWS and GCP
Comfortable with Docker, both running things locally and deploying them
Hands-on experience building or working with AI agents
Comfortable using AI coding agents (Cursor, Claude Code, OpenCode or similar) as part of your daily workflow, you ship faster because you use these tools well
Track record of owning something end-to-end: designing it, building it, and keeping it running in production
Comfortable reviewing code and setting technical standards, not just writing your own
Nice to have
Experience with EHR integrations, claims/billing systems, or healthcare APIs
Early-stage startup experience wearing multiple hats
Healthcare or RCM background
Prior experience as a forward-deployed engineer, solutions engineer, or similar embedded engineering role
