• New-patient visits: $75–$100 per completed visit.
• Established-patient E&M visits: $35–$60 per completed visit.
• RPM/CCM and other agreed clinical work: $75–$95 per hour.

About the role
We’re looking for a Texas-licensed nurse practitioner to join our remote clinical team as an independent contractor. You’ll primarily care for older adults with multiple chronic conditions, including those transitioning home after a hospital or post-acute stay.
You’ll initially focus on new-patient visits and well-defined clinical escalations during agreed coverage windows. This is a hands-on clinical role for someone who makes sound decisions independently, documents thoroughly, communicates clearly, and is comfortable working in an evolving, technology-enabled care model.
What you’ll do
• Conduct remote new-patient visits: review available records, evaluate the patient, reconcile medications, establish an assessment and plan, place appropriate orders, and complete and sign documentation.
• Triage and resolve clinical escalations involving abnormal vital signs or symptoms, medication questions and refills, urgent clinical review, care plans, and home health, therapy, DME, or referral needs.
• Prescribe or refill medications, sign orders and care plans, and direct patients to urgent or emergency care when clinically appropriate, within your credentialed scope and applicable law.
• Close the loop with patients, caregivers, Care Navigators, and referring partners so the outcome, responsible clinician or team member, and required follow-up are clear.
• Provide assigned follow-up evaluation and management visits and approved remote patient monitoring (RPM) and chronic care management (CCM) services.
• Collaborate with Care Navigators, who support preparation, coordination, and non-clinical follow-up, while remaining accountable for your clinical decisions and documentation.
• Document consistently in the practice’s electronic health record, use designated team communication tools, and identify opportunities to improve workflows and patient safety.
Coverage & response expectations
You will provide coverage on three mutually agreed weekdays each week, from 8:00 a.m. to 6:00 p.m. Central. You may perform other work during these windows, provided you remain reachable and able to begin appropriate clinical action within the required response target:
• High: Within 15 minutes. Needs immediate clinical review; emergencies follow the emergency protocol immediately.
• Medium: Within 1 hour. Needs prompt clinical review but can safely wait briefly.
• Low: Within 24 hours. Routine clinical requests that can safely wait.
These are initial-response targets; a complex case may take longer to resolve. Its status, next action, and responsible clinician or team member must remain clear.
Response obligations apply during contracted coverage windows. Before coverage ends, unresolved issues must be resolved or handed off to the designated covering clinician. Clinical leadership will arrange backup coverage for urgent issues when you are engaged in a patient visit or otherwise unable to respond within the required timeframe.
During the initial phase, you will focus on new-patient visits and well-defined high or medium escalations. Clinical leadership will retain routine or evolving workflows until the processes are established and you have completed the necessary training.
What success looks like
• Visits are clinically sound, completed on time, fully documented, signed, and ready for billing.
• Assigned escalations receive a timely clinical response and are safely resolved, transferred, or escalated with a clear handoff.
• Patients, caregivers, and Care Navigators receive clear clinical direction and timely follow-through.
• By the end of a structured 30-day ramp, you independently manage the agreed clinical responsibilities and meet the practice’s quality and documentation standards.
Required qualifications
• Board-certified family nurse practitioner or adult-gerontology primary care nurse practitioner; another primary care NP certification may be considered if scope and experience align.
• Current, unrestricted RN and Texas APRN licensure or applicable authorization to practice, plus an active individual NPI.
• Already enrolled—or able to promptly enroll—with Medicare and complete any required reassignment to the applicable Austin Health Wired-affiliated professional entity.
• Able to meet applicable physician-delegation, prescriptive-authority, credentialing, and professional-liability requirements before delivering care.
• At least 3 years of outpatient primary care, complex care, geriatric, home-based, or post-acute clinical experience.
• Strong E&M documentation, medication reconciliation, telehealth assessment, clinical triage, and interprofessional handoff skills.
• Comfort working independently within clear protocols, learning new technology, and adapting as workflows evolve.
Preferred experience
• Caring for Medicare patients with multiple chronic medical and social needs.
• Transitions of care, home health, RPM, CCM, or value-based care.
• Signing care plans or home-health orders and coordinating across patients, caregivers, and distributed care teams.
Engagement & payment details
This is a remote 1099 independent-contractor engagement. Specific coverage days and final compensation rates will be agreed upon before your start date.
Coverage pay applies to all contracted coverage hours, including time spent performing clinical work. Per-visit payments are earned once the encounter is completed, documented, signed, and ready for billing. Visit fees include associated record review, documentation, and orders; payment does not depend on claim submission or collection.
Documented RPM/CCM services and other agreed clinical work—including clinical escalations outside a paid visit—are paid at the agreed hourly rate. Time included in a visit payment is not paid hourly. Necessary clinical work within the agreed scope is compensated even when it does not result in a separately billable service.
The practice arranges and funds the required physician-delegation arrangements and professional-liability coverage for services performed under this engagement.
Onboarding & clinical support
You’ll work with Austin Health Wired’s clinical lead. During your first 30 days, you’ll learn the workflow, observe cases, perform visits and escalations with review, and progressively assume independent responsibility. Required onboarding and training are paid at the agreed hourly clinical rate.
The clinical lead will remain available for cases outside your assigned scope or requiring additional review. At the end of the initial ramp, we’ll review workload, visit duration, escalation volume, and the agreed scope together.
How to apply
Please submit your resume and a brief note describing your experience caring for older adults with complex needs and why Austin Health Wired’s mission is meaningful to you.
Austin Health Wired welcomes qualified candidates of every background. We do not discriminate on the basis of race, color, religion, sex, sexual orientation, gender identity, national origin, age, disability, veteran status, or any other characteristic protected by applicable law.
