Virtual Clinical Coordinator RN- Mount Sinai at Home- Full Time Days

Virtual coordination role to support front line clinical staff in care of patients receiving care in the home setting. Serve as an extension to front line staff in patient care logistics management including admission orders, admission equipment/education. Responsible for coordinating with clinical staff to ensure orders carried out, delivered, and vendor interfaces successful for order placement and completion. Support care of home based census of patients.

DESCRIPTION OF COMPETENCIES

A. Operational Duties
• Works as a member of our interdisciplinary care team assessing and addressing the complex care needs
of patients and families
• Performs visits for home based patients telephonically or via video to complete guided assessments,
support medication management, address symptoms and coordinate care
• Communicates plan of care and assessments to physicians, consulting clincians, and front line nursing teams
• Educates patient/family on trajectory of illness, use of medications, assesses disease understanding
• Participates in IDT meetings daily/weekly with the rest of the care teams
• Monitors care plans in conjunction with the rest of the clinical care team
• Documentation as required
• Performs other related duties as assigned

Required:

• Graduation from an accredited Nursing program. Bachelor’s Degree in Nursing. (BSN) Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year. Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO.
• Relevant clinical competence in area of nursing practice assigned; new graduates must possess current knowledge of the nursing process and its application
• Licensed as a registered nurse with current registration in New York State
• Name: Basic Life Saver (BCLS) Issuing Agency: AHA
• 2+ years patient experience
• Ability to work both day and evening shifts, rotating holidays, and on call
  Preferred:
• Spanish speaking proficiency/fluency
• Experience in the Emergency Departments, Intensive Care Unit or Med Surg.

Compensation Statement

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $87,692.00 - $131,538.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
