POSITION OVERVIEW: The Ambulatory Care Clinical Pharmacist provides expertise in a specialized healthcare team and for patients to assure optimal cost-effective drug therapy. This includes developing specific pharmacotherapeutic goals, selecting drug regimens and developing a monitoring plan to meet the individualized drug therapy needs of the patient. The Pharmacist is responsible for leading efforts in the prevention and reporting of medication errors and adverse drug reactions and works to continuously improve process and practice to enhance safety and efficacy of medication therapy. This position does not directly manage other caregivers. LOCATION: Pharmacists may be asked to work at multiple St. Charles Health System worksites at the request of leadership to fulfill short term, immediate staffing needs. ESSENTIAL FUNCTIONS AND DUTIES: Collaborates with health care providers to create, maintain, and utilize evidence based Collaborative Drug Therapy Management (CDTM) agreements to improve medication drug selection, monitoring, and patient adherence. Prescribes and manages specialty and other medications upon referral from St. Charles Licensed Independent Providers (LIP) as defined in CDTM agreements to assure optimal cost-effective drug therapy. Performs comprehensive medication and adherence management. Documents in the patient medical record and communicates with their referring LIP and the multidisciplinary team. Coordinates the removal of barriers to needed medication therapy; including prior authorizations, benefits investigation, prescription renewal, and financial assistance. Assists the multidisciplinary health care team with coordination of prescriptions, disease state education, lab ordering, and monitoring of patient response and adherence. Collaborates with pharmacy caregivers to ensure accurate and timely medication dispensing, refills, and deliveries. Monitors patient treatment (specialty and ancillary) medications for appropriateness, drug interactions, toxicities, and cost effectiveness based on current clinical and institutional guidelines as well as patient-specific criteria. Educates patients and their families to ensure their understanding of the goals, administration schedules and side effects of treatment. Participates in the training, education and mentoring of other pharmacists and technicians and precepting of pharmacy students and residents. Maintains liaison relationships with medical and nursing staff; provides timely information pertaining to pharmaceuticals, state and federal regulations regarding drug controls, and Joint Commission Standards. Provides supervision of assigned pharmacy technicians engaged in pharmacy practice. Ensures compliance with federal, state, and local laws and regulations and the maintenance of required records. Extracts pertinent information from patient/other sources for the patient's medical management plan. Operates pharmacy computer system(s) and other equipment. Assists with development of collaborative practice agreements, policies, and procedures. Maintains flexibility and readiness to work in multiple worksites either at the request of, or in mutual agreement with, leadership. Delivers customer service and/or patient care in a manner that promotes goodwill, is timely, efficient, and accurate. May perform additional duties of similar complexity within the organization, as required or assigned. EDUCATION: Required: Graduate of an ACPE accredited College of Pharmacy. Preferred: Doctor of Pharmacy or other advanced degree. LICENSURE/CERTIFICATION/REGISTRATION: Required : Current license or eligibility to be licensed as Pharmacist by Oregon Board of Pharmacy. ASHP Specialty Pharmacy Fundamentals Certification or other relevant certification for applicable disease state within 6 months of hire. AHA Basic Life Support for Health Care Providers Certification. Preferred : BCACP (Board Certified Ambulatory Care Pharmacist), BCPS (Board Certified Pharmacotherapy Specialist). Board of Pharmaceutical Specialties Certification in area of specialty practice. EXPERIENCE: Required : Completion of an ASHP accredited PGY-1 residency training program, or 5 years relevant clinical pharmacy experience. Preferred : Specialty / Complex Care / Ambulatory Pharmacy experience; Completion of PGY-2 residency. PERSONAL PROTECTIVE EQUIPMENT: Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely. PHYSICAL REQUIREMENTS: Continually (75% or more): Standing and walking, carrying 1-10 pounds, operation of a motor vehicle for caregivers that are couriers. Frequently (50%): Lifting 1-10 pounds, pushing or pulling 1-25 pounds, grasping/squeezing. Occasionally (25%): Sitting, bending, climbing stairs, lifting and carrying 11-25 pounds, Use of clear and audible speaking voice and ability to hear normal speech level. Rarely (10%): Keyboard operation, stooping/kneeling/crouching, reaching overhead, pushing or pulling 25-50 pounds. Never (0%): Climbing ladder/stepstool, lifting or carrying 25-50 pounds, whispered speech level, operation of a motor vehicle. Exposure to Elemental Factors Rarely (10%): Heat, cold, wet/slippery area. Never (0%): Noise, dust, vibration, chemical solution, uneven surface.
