Is this job info correct?

Make a lasting impact on patients living with complex rheumatologic disease through longitudinal, clinic-based pharmaceutical care. At UVA Health, ambulatory pharmacists are trusted clinical partners. This newly created Ambulatory Clinical Pharmacist embedded within Rheumatology represents a strategic investment in specialty care, patient access, and relationship-based practice. This role is designed for a pharmacist who wants to practice at the top of their license, build long-term patient relationships, and help shape how specialty pharmacy services integrate directly into outpatient clinical care. The Role This is a newly created, clinic embedded ambulatory clinical pharmacist position supporting UVA Health’s rheumatology physician group. While the role is aligned with UVA Specialty Pharmacy , most of the work is clinic based , partnering closely with rheumatology providers, nurses, and patients to manage complex medication therapy across the continuum of care. You will complement an existing rheumatology pharmacist and help expand services, capacity, and clinical impact as the program continues to grow. Serve as a core clinical partner to rheumatology physicians, advanced practice providers, nursing, and clinic staff. Provide longitudinal ambulatory pharmaceutical care , supporting patients over time rather than episodic encounters. Design, implement, assess, monitor, and document evidence based therapeutic plans for patients with complex rheumatologic conditions. Play a meaningful role in medication access , including specialty pharmacy workflows, prior authorization support, and limited distribution of drug access. Support specialty pharmacy accreditation related processes that directly impact patient access to therapy. Provide patient education, counseling, and ongoing medication monitoring , fostering trust and adherence. Participate in collaborative practice agreements , including ordering labs and medications when applicable. Potentially support comorbid condition management or disease state care within scope and training. Document care plans and clinical interventions clearly within the medical record. What an average day looks like Rounding or coordinating with rheumatology providers and clinic teams. Meeting patients for medication education, follow-up , and therapy optimization. Addressing medication access barriers and coordinating specialty pharmacy services. Reviewing labs, monitoring therapy effectiveness and safety, and adjusting plans under collaborative agreements. Serving as the primary embedded pharmacist in clinic at times, working independently while remaining deeply connected to the broader pharmacy and specialty teams. Why UVA Health UVA Health offers pharmacists the opportunity to practice in a highly collaborative, academic health system where innovation, professional growth, and patient centered care are deeply valued. This role is strategically important as UVA Health continues to expand specialty pharmacy integrated ambulatory care models , particularly in rheumatology. Why pharmacists choose UVA Health for roles like this Academic medical center environment that values interdisciplinary collaboration and evidence-based practice. A pharmacy friendly culture that encourages pharmacists to practice at the top of their license. Strong leadership support for collaborative practice agreements and expanded clinical scope. A specialty pharmacy program focuses on patient access, quality, and accreditation excellence . Opportunity to help build and shape a growing clinic embedded pharmacist model , not simply step into an existing template. Clear pathways for professional growth and advancement through the clinical ladder , with multiple pharmacists having successfully progressed. A role designed for meaningful clinical impact, not task based or technician focused work. Relocation assistance may be available for qualified candidates.
