Are you passionate about improving outcomes for people with disability and creating better connections between health and community support services? At Central Coast Local Health District (CCLHD), we are committed to Trusted Care. Better Health for Everyone, and we are seeking a Disability Pathway Navigator to provide specialist advice, system leadership and support at the interface between healthcare services and the National Disability Insurance Scheme (NDIS).

About The Opportunity

As a Disability Pathway Navigator, you will play a key role in supporting people with disability, their families and carers to access appropriate disability and health services. Working across the District, you will provide expert guidance on NDIS-related matters, develop pathways and processes, and collaborate with stakeholders to improve service experiences and outcomes for people with disability.

Role

This position plays an important role in building capability across CCLHD and strengthening the interface between health services and the disability sector. In this role you will:

  • Provide specialist advice and guidance on NDIS processes, pathways and disability-related matters to support staff, consumers and stakeholders.
  • Facilitate complex clinical and systemic escalations, identifying solutions and developing strategies to address barriers to care and service access.
  • Build and maintain effective partnerships with internal and external stakeholders to improve coordination and service delivery outcomes.
  • Lead the development and continuous improvement of systems, processes, reporting and workforce capability relating to disability inclusion and the NDIS.
  • Analyse and report on disability and NDIS-related issues, risks and trends to inform decision-making, service planning and improvement initiatives.

Applicants interested in part time are encouraged to apply.

For more information about this role, please view the Position Description.

About You

We are looking for someone who has:

  • Experience working within the disability, health or community services sector, with a strong understanding of the NDIS and disability support pathways.
  • Demonstrated ability to build collaborative relationships and work effectively with a diverse range of stakeholders.
  • Strong analytical, problem-solving and project management skills with the ability to manage complex issues and competing priorities.
  • Experience developing policies, pathways, reporting frameworks or service improvement initiatives.
  • Excellent written and verbal communication skills, with the ability to provide high-level advice and prepare reports, briefings and correspondence.

Applicants must have current work rights in Australia or be a permanent resident or citizen. Unfortunately, we cannot consider applicants who do not meet this requirement.

Benefits

  • Work-Life Balance: Enjoy an accrued day off each month, 17.5% annual leave loading, and paid parental leave.
  • Financial Benefits: Boost your take-home pay with salary packaging, save on car costs with novated leasing, and access relocation assistance.
  • Health & Wellbeing: Stay active with discounted gym memberships (Fitness Passport), free flu vaccinations, telehealth support, and confidential Employee Assistance Program (EAP) counselling for you and your family. Plus, access discounted private health insurance.
  • Career Growth: Advance your career with free professional development courses and secondment opportunities.
