Overview: The Client Care Coordinator supports the coordination and continuity of care for clients participating in Bellwood’s specialized programs. Working closely with Clinical Managers, clinical teams, clients, and referral partners, the Client Care Coordinator monitors service and reporting timelines, coordinates communication, and helps resolve issues affecting client experience. This role focuses on client care coordination, stakeholder support, and timely service delivery. Key Responsibilities: Coordinate communication among clients, clinical teams, and referral partners to support timely and consistent care. Support clients throughout admission, treatment, discharge, and transition planning. Monitor service commitments and reporting timelines. Follow up with clinical team members when information is outstanding. Identify and escalate client concerns, service gaps, scheduling conflicts, and reporting delays. Coordinate program activities, case conferences, clinical schedules, and room requirements with the appropriate teams. Maintain regular communication with referral partners and provide updates on service delivery within the scope of the role. Collaborate with Clinical Managers and care teams to address client and program needs. Track service issues and identify opportunities to improve coordination and client experience. Maintain client privacy and confidentiality in accordance with legislation and EHN Canada policies. Participate in quality improvement activities and perform other client care coordination duties within the scope of the role. Qualifications: Post-secondary education or equivalent relevant experience. Additional training or certification in medical or mental health is an asset. Strong organizational skills and attention to detail. Minimum of 2 years' experience in a coordination role. Experience in healthcare, mental health, or a similar field is considered a strong asset. Proficient in Microsoft Office Suite (Excel, Word, Outlook, PowerPoint). Experience with Electronic Medical Records (EMR) systems is an asset.
