Director of Case Management Location: Northwestern Arizona on the Colorado River, only 80 miles from Las Vegas! Key Responsibilities: As the Director of Case Management , you will play a pivotal role in our hospital's strategic plan by: Conducting comprehensive assessments of individual patient needs. Developing and implementing patient care plans. Overseeing utilization review and management of discharge planning. Ensuring the highest level of patient satisfaction and continuity of care. Coordinating and managing the care and discharge planning for all patients. Qualifications: Education: Graduate of a Program in Nursing at the Bachelor of Science level or higher (Master's degree preferred). CCM or ACM Certification is preferred . Skills: Critical thinking and problem-solving abilities. Strong interpersonal and communication skills. Proficiency in data analysis, report generation, and budget management. Organizational leadership and management skills. Work Experience: A minimum of five (5) years of management experience. Five (5) years of utilization review/case management experience in an acute short-term hospital. Five (5) years of experience in discharge planning in an acute short-term hospital. Experience with Interqual or MCG criteria software, Microsoft Word, Excel, and PowerPoint. Preferred experience with MedHost EMR and MorCare. Licenses: Registered Nurse with current RN Licensure from the State of Arizona or compact license accepted by the State of Arizona. Basic Life Support (BLS) certification. You'll have the opportunity to sit for the appropriate certification within 30 months of hire, provided all other criteria are met.
