Job Overview
We are seeking an experienced Occupational Therapist to join our developing Community Integrated Stroke Team in Ceredigion, supporting people who have predominantly experienced a stroke, to return home from our acute stroke and rehabilitation units.
If you have a passion for stroke rehabilitation, combined with community working, this could be the opportunity for you. The role is focussed on delivering rehabilitation at home and in the local community, to support transition from hospital, ongoing recovery, and return to independence.
The post holder will work as part of a multi-professional team, working closely with our acute stroke teams and wider community services, to deliver person-centred care and a rehabilitation pathway, that ensures continuity and a positive experience for our population.
Successful applicants will have some experience of working with people who have experienced stroke and/ or other life changing events and enjoy working as part of a multi-professional team, within a community setting. Whilst the role focusses on stroke, we require therapists who are adaptable to meet needs of patients with multiple and varying conditions, as well as service priorities.
You will have the support of an occupational therapy service which values supervision, actively promotes continuous professional development, and is committed to excellent patient care and service improvement.
Main duties of the job
As an Occupational Therapist, you will autonomously manage an occupational therapy caseload of a designated client group, which includes those with complex presentations. This will involve selecting and undertaking assessments, developing person centred plans, delivering interventions, evaluating outcomes and adapting plan of care based on outcomes.
You will work effectively as part of a multidisciplinary team to provide rehabilitation/recovery/reablement focussed on the needs of the individual.
Working collaboratively with colleagues from Health, Local Authorities and Third Sector Organisations to achieve best outcomes, you will support the induction, education and development of others and support quality improvement, taking responsibility for defined projects.
You will appropriately delegate work to, and supervise the work of, Occupational Therapists, Support Workers, Students and Volunteers as required.
Due to the nature of the role you will need to be able to travel on a daily basis within the County, as well as being flexible to travel across the Health Board on occasions. Mileage allowance is payable and a lease car scheme is available.
