AI Product Engineer (Clinical AI) - Start Up
- $200,000 to $245,000 plus competitive equity
- New York, hybrid with 3 days onsite
- Open to visa transfers including OPT and H1B
I'm working with a seed-stage clinical AI company in New York building the execution platform for risk-bearing care organisations. Their multi-agent system works both sides of the doctor visit off a shared patient context graph: clinician-side agents read the chart and surface specialist-level recommendations inside the EHR, while patient-side agents reach out between visits to gather updates and triage concerns, handing off to a human the moment something needs judgement. It's live in production and already showing 3x faster chronic condition control. They're hiring an AI Product Engineer to own that agentic platform end to end.
The Role
You'd be the person who owns the agent platform, working directly with the founders and a small engineering, AI and clinical team. This is genuine end-to-end ownership, from early prototype through to software real clinicians use every day.
Typical work includes:
- Building and evolving the agentic platform: context management, tool use, orchestration, permissions, memory and human-in-the-loop workflows
- Prototyping new clinical AI experiences quickly and turning the strongest into production software
- Designing evaluations, regression suites and launch criteria covering task success, clinical quality, reliability, safety, latency and cost
- Building observability across model behaviour, tool calls, traces and end-to-end workflow outcomes
- Working closely with clinicians and product to turn real clinical workflows into trustworthy software
Tech Stack
Python, TypeScript, LLMs, agentic AI frameworks, RAG and retrieval systems, APIs, backend services. Modern React-based frameworks are a plus.
What they're looking for
- 3+ years in product engineering with a focus on LLM-based systems
- Agentic or LLM systems you've taken to production, covering tool calling, orchestration and context management
- Experience conceptualising, architecting and building a product end to end
- Python and TypeScript across backend and product surfaces
- Experience with LLM evaluations, regression testing or AI observability tooling
- Healthcare or another highly regulated domain
- Able to explain technical tradeoffs clearly to clinicians and other non-technical people
The Opportunity
- Own the agentic clinical platform outright rather than a slice of it, reporting into the Head of Engineering and working alongside the founders
- Applied AI pointed at chronic disease care for rural and underserved patients, live in production with real clinical outcomes behind it
- Small team of around 20, backed by well-known healthcare and AI investors, with leadership from some of the biggest names in value-based care
- $200,000 to $245,000 plus competitive equity
- Open to visa transfers including OPT and H1B
