FHCSD Program for All-inclusive Care for the Elderly (PACE) is designed to
help our patients maintain a high standard of independent living.
Our program provides a wide range of supportive services including in-home care, meals, medical equipment, transportation, social services, and social activities. We are currently seeking a dedicated Nurse Practitioner or Physician Assistant to join our team at FHCSD PACE College Grove. In this position you will act as a crucial member of an integrated health team that is dedicated to providing well-coordinated and high-quality care to all of our participants.

Duties and Responsibilities
Complete all required documentation in accordance with department standards
Work effectively with patients from diverse social, cultural and economic groups
Participate in PACE Interdisciplinary Team (IDT) meetings
Maintain all necessary certifications and licensures required
Attend all mandatory meetings; minimizing incident reports; using resources efficiently and practicing medicine in a cost-effective manner.
Maintain FHCSD behavioral standards, including (but not limited to), effective listening and communication skills and respectful interactions with colleagues and patients.
Maintain satisfactory peer reviews: able to assess medical information including pathophysiology, patient presentation, differential diagnosis, etc., in order to facilitate health promotion and disease prevention.
Meet clinic site and organization needs in a reliable and flexible manner; prioritizes work and makes an effort to adhere to schedule.
Participate in Clinical Outcomes Program and work towards meeting organizational targets for all metrics.
Participate in administrative duties, including lab reports, charts, and medication refills reviewed and managed appropriately.
Perform other duties as assigned.
Requirements
Minimum of 2 years of experience working with underserved, low-income older adult persons in an outpatient clinic or similar setting required.
Completion of an approved/accredited Physician Assistant or Nurse Practitioner program
Current unrestricted license to practice in the State of California or ability to obtain
DEA license or ability to obtain
Current American Heart Association health care provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training
Must meet FHCSD Driving Requirements
Second language fluency desired
