Learn more at: Centerstone Medical Group - Centerstone Job Summary As a Nurse Practitioner with Centerstone Medical Group, you will provide comprehensive, patient-centered healthcare to individuals with complex medical, behavioral health, and substance use disorder needs. This role combines primary care and addiction medicine, providing assessment, diagnosis, treatment planning, medication management, and ongoing care coordination. The Nurse Practitioner works collaboratively with a multidisciplinary healthcare team to promote continuity of care, support positive health outcomes, and ensure high-quality services for the individuals we serve. The ideal candidate is mission-driven, adaptable, compassionate, and committed to serving diverse and underserved populations. Essential Job Functions Provide comprehensive, patient-centered care through physical and psychological assessments, detailed health histories, and age-appropriate evaluations. Evaluate, diagnose, and develop individualized treatment plans using evidence-based clinical practices. Prescribe and manage pharmacologic and non-pharmacologic interventions within the provider's scope of practice and applicable laws and regulations. Monitor patient progress and treatment effectiveness, adjusting interventions as clinically appropriate to promote positive outcomes. Provide medication services within Substance Use Disorder programs, including mental status examinations, symptom assessments, medication education, and monitoring for adverse effects. Coordinate ongoing patient care and support continuity of services across healthcare settings and programs. Collaborate with multidisciplinary team members to support patient transitions, coordinate referrals, and develop comprehensive care plans. Participate in daily team huddles and other care coordination activities to promote effective communication and continuity of care. Coordinate with external healthcare providers and organizations to obtain relevant medical records, test results, and other information necessary for patient care. Develop, implement, and approve standing orders in accordance with applicable state laws, regulations, and Centerstone Medical Group guidelines. Provide education and support to patients regarding diagnoses, treatment options, medications, wellness, and preventive care. Maintain accurate, timely, and complete documentation within the electronic health record. Stay current with evidence-based practices and emerging approaches in primary care, addiction medicine, behavioral health, and related areas of practice. Demonstrate cultural sensitivity and provide compassionate care to individuals from diverse ethnic, cultural, and socioeconomic backgrounds. Collaborate effectively with other providers and healthcare professionals to support an integrated model of care. Participate in required meetings, trainings, and professional development activities. Perform other duties as assigned. Knowledge, Skills, and Abilities Strong clinical judgment and the ability to independently assess patient needs and develop appropriate treatment plans. Effective written and verbal communication skills, with the ability to communicate complex medical information clearly to patients, families, providers, and other healthcare professionals. Ability to establish and maintain effective professional relationships with patients, healthcare personnel, community partners, and external providers. Ability to work effectively as part of a multidisciplinary and integrated healthcare team. Ability to demonstrate compassion, professionalism, and cultural sensitivity when working with diverse patient populations. Ability to adapt to changing patient needs, clinical environments, and organizational priorities. Ability to read and interpret clinical guidelines, policies, procedures, and other professional documents. Ability to accurately document patient care and communicate effectively through an electronic health record and other electronic systems. Ability to exercise sound professional judgment and maintain appropriate professional boundaries. Ability to provide telehealth services and effectively engage patients through virtual care platforms. Experience and Education Qualifications Graduate of an accredited school of nursing and an accredited Nurse Practitioner program. Current Registered Nurse (RN) license in the state of practice. Current Advanced Practice Registered Nurse (APRN) license in the state of practice. Minimum of two years of experience as a Nurse Practitioner. Must maintain all required professional licenses, certifications, and credentials in good standing throughout employment. Federal DEA registration required. Willingness to explore and/or obtain applicable federal authorization or certification required for the treatment of individuals with substance use disorders, as applicable. Current driver's license, acceptable driving record, and current auto insurance. Willingness to obtain additional state licensure as needed to support organizational operations. Willingness and ability to provide telehealth services.
