Role: Physical Therapist providing in-home, senior living, and virtual care to older adults, focusing on improving mobility, independence, and quality of life.
Responsibilities: Conduct evaluations, develop individualized plans, deliver evidence-based therapy, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications: Valid state license or eligibility, degree from an accredited program, basic computer skills, and current CPR certification.
Physical Requirements: Ability to perform moderate physical tasks, assist with patient transfers, stand/walk for extended periods, and drive to patient locations.
