Role Overview: A licensed Physical Therapist providing personalized, one-on-one care to older adults in private residences and senior living communities.

Key Responsibilities: Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life. Document treatment progress in electronic medical records, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff on safety and aging-in-place strategies. Incorporate telehealth and virtual care to enhance access and flexibility.

Qualifications & Skills: Valid state license, degree from an accredited program, proficiency in EMR and basic computer skills, and current CPR certification. Ability to perform physical tasks including lifting, repositioning, and assisting with patient transfers. Must meet physical demands of in-home and community care, including driving to patient locations.
