We are seeking a Physical Therapist to provide outpatient rehabilitation services primarily in home settings for older adults. The role involves evaluating, developing, and implementing individualized therapy programs to restore and enhance patient function, as well as educating patients and caregivers to promote independence. The therapist will supervise support personnel as needed, ensure timely documentation, and collaborate with the rehabilitation team to tailor treatment plans.

Assess patients through history, tests, and measures to establish diagnoses and treatment goals.
Deliver therapeutic techniques including exercise, manual therapy, modalities, and functional training.
Design and fit adaptive devices, instruct patients on mobility and device use, and modify plans based on progress.
Perform home and community mobility assessments and provide safety recommendations.
Ensure a safe environment, order supplies, and maintain equipment.
Contribute to team meetings, referrals, and case management efforts.

Qualifications: Must hold a valid Physical Therapy license or license eligibility, with a Master’s degree in Physical Therapy or related field, or a Bachelor’s with extensive experience. A valid driver’s license and reliable transportation are required.
