Physical Therapist – Summerville, SC
Seeking licensed Physical Therapists passionate about delivering personalized, one-on-one care to older adults in private residences or senior living communities.
Utilize the Geriatric House Calls™ model to improve patients' mobility, independence, and quality of life through evidence-based treatments.
Key responsibilities include conducting evaluations, creating individualized care plans, delivering therapy sessions, and documenting in electronic medical records.
Collaborate with interdisciplinary teams of PTs, OTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff to promote safety and aging in place. Telehealth and virtual care options support flexible scheduling and extended access.
Qualifications require a valid PT license, degree from an accredited program, basic computer skills, and CPR certification. Physical demands involve assisting with patient transfers, repositioning, and patient mobility activities, including driving to patient locations.
