Role: Licensed Physical Therapist providing personalized, in-home and community-based care to older adults, including virtual services.
Responsibilities: Conduct evaluations, develop individualized treatment plans, deliver evidence-based therapy to improve mobility, independence, and wellness. Document sessions in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and utilize telehealth when appropriate.
Qualifications & Skills: Valid state license, degree from an accredited program, proficiency with EMR and computers, current CPR certification. Physical stamina to perform transfers, repositioning, and extended walking; ability to lift up to 50 lbs; and willingness to drive to patient locations.
