Role Overview: The Physical Therapist provides outpatient rehabilitation services primarily in patients' homes or assisted living facilities, focusing on older adults. They evaluate, develop, and implement personalized therapy programs to restore and enhance mobility, strength, and independence. The role involves patient assessments, designing treatment plans, instructing patients and caregivers, and coordinating with other healthcare professionals to address comprehensive patient needs.

Key Responsibilities

Assess patients through examination, history, and tests to establish diagnoses and treatment goals.
Develop and modify individualized therapy plans, utilizing techniques such as therapeutic exercises, manual therapy, functional training, and use of modalities.
Educate and train patients and caregivers to promote independence and safety.
Supervise support personnel and collaborate with the rehabilitation team.
Order supplies, maintain equipment, and ensure a safe environment.
Perform home and community mobility assessments and recommend adaptive devices.
Document clinical activities, adhere to schedules, and participate in meetings.

Qualifications & Skills

Valid Physical Therapy license in applicable state(s).
Master’s degree in Physical Therapy or related field, or equivalent experience.
Reliable transportation and a valid driver’s license.
