We are seeking a Full-time Physical Therapist to provide outpatient rehabilitation services primarily in in-home settings such as private residences and independent living facilities.

- Evaluating, developing, and delivering individualized physical therapy programs to promote rehabilitation and independence.
- Assessing patient needs, making referrals to other disciplines, and adapting treatment plans accordingly.
- Instructing and training patients and caregivers on essential skills and functions.
- Supervising support personnel, ensuring adherence to schedules, documentation, and safety protocols.
- Performing comprehensive evaluations, therapeutic interventions (e.g., therapeutic exercises, manual therapy, modalities), and functional assessments.
- Designing and fitting adaptive devices, and providing mobility assessments and recommendations.
- Promoting health, wellness, and fall prevention strategies in community and home environments.
- Valid Physical Therapy license or eligibility in the state(s) of practice.
- Master’s degree in Physical Therapy or equivalent; or a Bachelor’s degree plus five years of experience.
- Reliable transportation and a valid driver’s license.
