We are seeking a Full-time Physical Therapist to provide outpatient rehabilitation services primarily in in-home settings such as private residences and independent living facilities.

Key Responsibilities Include

Evaluating, developing, and delivering individualized physical therapy programs to promote rehabilitation and independence.
Assessing patient needs, making referrals to other disciplines, and adapting treatment plans accordingly.
Instructing and training patients and caregivers on essential skills and functions.
Supervising support personnel, ensuring adherence to schedules, documentation, and safety protocols.
Performing comprehensive evaluations, therapeutic interventions (e.g., therapeutic exercises, manual therapy, modalities), and functional assessments.
Designing and fitting adaptive devices, and providing mobility assessments and recommendations.
Promoting health, wellness, and fall prevention strategies in community and home environments.

Qualifications

Valid Physical Therapy license or eligibility in the state(s) of practice.
Master’s degree in Physical Therapy or equivalent; or a Bachelor’s degree plus five years of experience.
Reliable transportation and a valid driver’s license.
