Join a patient-centered rehabilitation team as a full-time Physical Therapist, providing outpatient services primarily in in-home settings for older adults. Your responsibilities include evaluating, developing, and implementing individualized therapy programs to promote independence, restoring mobility, and enhancing overall well-being. You'll collaborate with other healthcare professionals, supervise support staff, and adapt treatment plans based on patient needs. Additional duties involve patient education, recommending assistive devices, conducting functional assessments, and ensuring a safe environment.
Qualifications include a valid Physical Therapy license, a Master's degree in Physical Therapy or equivalent, and reliable transportation. Experience in outpatient and home-based care is preferred.
