Physical Therapist

Provides outpatient rehabilitation services to older adults in home, ALF/ILF, and community settings, focusing on functional assessment, development, and implementation of personalized therapy programs aimed at restoring and maintaining independence.

Responsibilities include evaluating patients, creating treatment plans, instructing patients and caregivers, and making referrals to other disciplines as needed. The role involves supervising support personnel, ensuring documentation compliance, and maintaining equipment and safety standards.

The position requires valid licensure in the state, a Master’s degree in Physical Therapy or equivalent, and reliable transportation. The role emphasizes patient-centered care, health promotion, and functional mobility training, including manual therapy, therapeutic exercises, and adaptive device instruction.
