Role Overview: A licensed Physical Therapist provides personalized, one-on-one physical therapy to older adults in various settings, including private residences and senior living communities. The position emphasizes autonomy, flexibility, and meaningful patient outcomes, allowing therapists to create and manage their own treatment schedules and incorporate telehealth into care delivery.

Key Responsibilities

  • Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy sessions to support mobility, independence, and wellness.
  • Document patient progress accurately within the EMR system, adhering to regulatory standards.
  • Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
  • Educate patients, caregivers, and staff to promote safety and aging in place.
  • Utilize telehealth/virtual care when appropriate to enhance access and continuity.

Qualifications & Skills

  • Valid Physical Therapy license or eligibility in the state of practice.
  • Degree from an accredited PT program.
  • Proficiency in basic computer and EMR use.
  • Current CPR certification.

Physical Requirements: Ability to lift 10–50 lbs, provide patient assistance, perform CPR, and stand or walk for extended periods. Must be able to drive to patient locations and wear PPE as needed.
