Role Overview: We are seeking licensed Physical Therapists to provide one-on-one geriatric care in patients' homes and senior living communities. The position emphasizes autonomy, flexibility, and meaningful patient outcomes, with options for in-person and telehealth services.

Key Responsibilities

Perform evaluations, develop individualized care plans, and deliver evidence-based therapy sessions to enhance mobility, independence, and safety.
Document patient progress accurately within the EMR system, complying with regulatory standards.
Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
Educate patients, caregivers, and staff to promote safe aging-in-place practices.
Utilize telehealth when appropriate to extend care access and flexibility.

Qualifications & Skills

Valid state license in Physical Therapy or eligibility to obtain one.
Degree from an accredited program and proficiency in EMR systems.
Current CPR certification.
Ability to meet physical demands such as lifting, repositioning, and providing assistance with patient transfers.
