- Perform evaluations, develop individualized care plans, and deliver evidence-based therapy sessions to enhance mobility, independence, and safety.
- Document patient progress accurately within the EMR system, complying with regulatory standards.
- Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff to promote safe aging-in-place practices.
- Utilize telehealth when appropriate to extend care access and flexibility.
- Valid state license in Physical Therapy or eligibility to obtain one.
- Degree from an accredited program and proficiency in EMR systems.
- Current CPR certification.
- Ability to meet physical demands such as lifting, repositioning, and providing assistance with patient transfers.
