Board certification in Internal Medicine or Family Medicine (Geriatric Medicine certification preferred). Active and unrestricted medical license in California. Demonstrated experience in post-acute, skilled nursing, long-term care, or geriatric settings. Strong clinical skills in managing complex, multi-morbid patients. Excellent communication and interpersonal skills, with a patient- and team-centered approach. Ability to work effectively in an interdisciplinary, facility-based environment. Proficiency in electronic medical records (EMR) systems. Preferred Qualifications: Fellowship training in Geriatric Medicine or experience in post-acute/rehabilitation medicine. Knowledge of CMS regulations, SNF billing and documentation requirements, and quality metrics. Experience participating in value-based care or population health programs.
